Mat-Su Health Services is seeking an Open Access Case Manager to join our team!

Summary: Under the general supervision of the Rehab Services Manager, the Open Access (OA) Case Manager provides daytime face to face or telephonic screening for level of care recommendation, triage and clinical intervention for individuals in crisis. May provide outreach, follow-up, advocacy, and referral to community resources for individuals pursuing behavioral health services at MSHS.

  • Maintains regular contact with outside resources and providers. Advocates on behalf of the consumer to assist qualifying clients/families in identifying, obtaining, and securing needed employment, entitlements, supports, and services to meet immediate needs.
  • Provide daytime phone and face-to-face psychiatric emergency response/triage by determining clinical acuity, resolving the clinical situation, or seeking further assistance from the open-access team.
  • Completes Pre-Admission screenings to prospective clients and determines the appropriate level of service.
  • Follows up on other agency referrals and contacts referred clients within established time frames.
  • Maintains accurate screenings and call logs, distributed to the appropriate recipients within the established time frame.
  • Maintains an up-to-date resource list for use by agency personnel.
  • Coordinates with the Open Access Team to ensure that the team is aware of high-risk/priority clients so they can be seen within the appropriate time frame.
  • Assists individuals in completing healthcare marketplace applications to obtain health insurance.
  • Provides and participates in interdisciplinary quality assurance, utilization review, treatment planning, treatment assessments, and function assessment.
  • Demonstrates competencies in client engagement, crisis intervention, documentation, and obtaining resources/entitlements for individuals seeking services.
  • At the direction of the Rehab Services Manager, trains Case Managers to complete screening and Open Access services and documentation when needed to maintain consistent coverage for the Open Access role.

Minimum Education and Experience:

  • A BA/BS degree in Social Work, Psychology, Human Services, or a related field, along with some work experience, is preferred; education and relevant work experience equivalent to five years may be considered.
  • Minimum of 3 three years in a Case Management or equivalent position.
  • Have general knowledge of EMR/EHR software and Microsoft Office programs
  • Ability to read and interpret documents such as safety rules, operations and maintenance instructions, and procedure manuals

General Requirements:

  • Clear background check (subject to DHSS background check)
  • Current driver’s license; clean driving record
  • Current tuberculosis screening test result
  • Current First Aid and CPR certifications. CPI (Nonviolent Crisis Intervention) certifications must be obtained when offered through the agency; may substitute current MANDT certificate for CPI.
