PART TIME 18 HOURS/ BWH BOSTON

GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Pediatric Nurse Practitioner (PNP) for the Center for Child Development is accountable for delivering comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice to infants in the Follow-Up Program. This person will function as a member of an outpatient multidisciplinary team within an integrated academic model.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Responsibilities

  1. Support a multidisciplinary team to promote optimal health and developmental outcomes for NICU or Special Care Nursery discharged infants who are enrolled in the Follow-Up Program.
  2. Communicate with patients' PCP.
  3. Communicate with patients' Early Intervention or outpatient therapy service providers.
  4. Coordinate follow-up with medical specialists when warranted.
  5. Monitor daily flowsheets from patients through Epic MyChart.
    1. Respond to patients' MyChart Messages.
    2. Assess patients' trends over the flowsheets and make clinical decisions appropriately.
  6. Manage Home Health Equipment orders and paperwork along with PCP.
  7. Assist in writing WIC prescriptions.
  8. Assist seeing patients in the Follow-Up Program on Mondays and Fridays.
    1. Perform pre-clinic preparation: including but not limited to comprehensive chart reviews, obtain clinical records, and prepare clinical summaries for patients attending the clinic.
    2. Assist families with completing clinic paperwork including developmental questionnaires prior to clinic visits.
    3. Clinic role includes: Perform ing history and physical examinations on new and follow up patients in clinic. Diagnose and manage conditions and prescribe necessary medications or treatments. Order and interpret necessary diagnostic and/or therapeutic tests. Provide anticipatory guidance regarding growth and development in medically complex children. Assess parental coping capacity and parent/child relationship. Educate and refer families to local, state, and/or and national support groups. Promote team-based care through communicating the primary care provider/pediatrician with any significant changes to the patient's medical and developmental plan following clinic.
  9. Assist the transition of medical and developmental services at key transition points. i.e. Follow-Up Program Preschool Program Transition to school/IEP and 504 reviews.

Leadership/Teaching

  1. Assist in supporting the education of medical students, residents, fellows, nurses, NPs etc. both inpatient and outpatient regarding discharge planning and coordination as well as on-going follow up.
  2. Serve on a leadership team in the Center for Child Development to promote its growth.
  3. Create CME programs for community primary care providers on how to support and manage the medical and developmental needs of children after NICU discharge.

Research

  1. Champion QI initiatives to evaluate the benefits of the Follow-Up Programs to overall health care spending, health outcomes, and developmental outcomes.
  2. Remain actively involved in the scientific community by presenting at conferences and poster presentations.

QUALIFICATIONS:

  1. Masters' (or) DNP (or) PhD Degree in Nursing required. Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as a nurse practitioner and is accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).
  2. Board certification as a Pediatric Nurse Practitioner required.
  3. Neonatal Resuscitation Program NRP certification required.
  4. Minimum one year current experience as a clinical nurse practitioner in a Primary Care environment caring for a pediatric patient population required.
  5. Minimum one year clinical nursing experience caring for a pediatric patient population strongly preferred.
  6. Proficiency in reading, writing, and speaking Spanish required.
