Job Description: This is a full-time registered nurse position. The incumbent is responsible for conducting assessments to determine eligibility for home care services, along with completing and submitting MDS-HC forms. This is a field-based position, which requires travel throughout Massachusetts.

Monday-Friday 8:30-4:30PM, No Weekends, No Holidays.

Requirements:

-Registered Nurse License in Massachusetts

- At least two years of recent experience in the direct care of the elderly or disabled adults. -Must be able to pass a criminal background check.

-Must have a valid driver's license.

-Have a vehicle in good working order and be willing to travel.
