What You'll Do:

As a Nurse Practitioner, you will carry out our Mission of Love by providing pain and symptom management expertise for Hospice Patients in collaboration with the Hospice Medical Director and IDG. The Hospice Nurse Practitioner will also provide Face to Face visits in order to meet the Hospice Conditions of Participation. In addition, this role will serve as a liaison to educate the community at large about end of life and palliative care.

Our Nurse Practitioner is responsible for the following:

  • Reviewing the patient's clinical information before the recertification period for each patient which includes a face to face visit by the medical director or designee before the third Benefit period and with each subsequent benefit period.
  • Serving as a consultant in collaboration with the Hospice Medical Director regarding pain control and symptom management.
  • Providing consultation service to the clinical staff on problems relating to the patient's medical care as needed.
  • Providing consultation concerning clinical protocols.
  • Attending Hospice IDG as able to discuss mutual patient problems with the Hospice IDG Team.
  • Documenting the patient's progress toward the outcomes specified in the Plan of Care.
  • Serving as a medical resource for the hospice interdisciplinary group and as a liaison to physicians in the community.
  • Developing and coordinating procedures for the provision of emergency care
  • Proving a system to assure continuing education for hospice clinical staff as needed.
  • Providing direction and education for field staff as needed
  • Attending Hospice QAPI Monthly as schedule allows.
  • Assisting with Palliative Care Symptom Management visits as the Palliative Care program is established.
  • Participating in an On-call Rotation for clinical phone consultation and occasional emergency Face to Face with Hospice Patients.

What You Have:

  • Must be an RN and hold a BSN in addition to an MSN Degree with a NP Focus and successfully pass the National Board certified NP Exam.
  • Currently licensed and legally authorized as a Nurse Practitioner (CRNP) to Practice medicine and prescribe pharmaceuticals and medical treatments in the State of Pennsylvania in a Collaborative Agreement with the Hospice Medical Director.
  • Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania requirements.
  • Two (2) years' experience in relating to hospice patients, skilled care and Palliative care as an RN or CRNP
  • Must be knowledgeable in the Hospice Medicare Conditions of Participation as well as all Federal, state and local regulations.
  • Participation in the Medicare and Medicaid programs.
  • Must be knowledgeable about the medical and psychosocial aspects of hospice care.
  • Demonstrates expertise in managing pain and other symptoms associated with terminal illness regardless of the setting in which the patient is receiving services.
  • Demonstrates knowledge of and a respect for the rights, dignity and individuality of all patients/caregivers served.
  • Develops and maintains cooperative working with hospice staff, other professionals and physicians in the community.
