POSITION TITLE: Special Education/Case ManagerQUALIFICATIONS: NH Certification in General Special Education, knowledge of Special Education regulations and timeline, experience as a classroom and/or special education teacher.REPORTS TO: Building Principal/Special Education CoordinatorSUPERVISES: Special Education ParaprofessionalsPERFORMANCE RESPONSIBILITIES•Implement and monitor Individual Education Plans•Evaluate students to identify needs and monitoring effectiveness of IEP•Coordinate specialized evaluations for qualifying students•Work with classroom teacher to ensure modifications are developed and implemented•Coordinate the delivery of related services for the student•Maintain and update a student’s special education file•Document and maintain all special education-related paperwork (WPNs, Meeting minutes, correspondence, etc) •Train staff regarding expectations of needed modifications of identified students•Coordinates and provides assisted technology•Schedule, coordinate, and effectively prepare for special education meetings•Facilitate IEP meetings using standard meeting protocols •Communicate with parents as necessary to ensure families are aware of concerns and successes.•Keep staff apprised of pertinent information regarding students on the caseload•Verify that each IEP remains current and anticipates need to address IEP due dates•Perform other such duties as may be required to ensure student success.
