Purpose:Mary Lee Foundation is a 501(c)(3) nonprofit organization dedicated to providing diverse services and housing to adults with various types of disabilities.

As the Case LVN you provide individualized, goal-directed nursing care to assigned clients with predictable healthcare needs within well-defined policies, procedures, and guidelines in which assistance and support are available from the clinical supervisor, Lead RN.This position is the front line for nursing and is integral in collecting data to help develop, modify, implement, and evaluate the responses to their nursing care plan. As the Case LVN, you will create and educate on consumer-specific healthcare topics and reinforce education provided by the RN to ensure that care is being provided appropriately in accordance with the individual’s Individual Plan of Care (IPC), Implementation Plan, Program Guidelines, and Texas State Guidelines.

Responsibilities:

• Will receive assignments from and operate under the supervision of the Lead RN.
• Complete in-person face-to-face and telehealth focused assessments.
• Complete interviews with individuals and participants in their care, i.e. group home staff, guardian, CRA, and Host Home Provider as needed during focused assessments.
• Complete review of medical records as needed.
• Check in daily arrivals of medications from pharmacy, reviewing the type and amount of medication, dosage instructions, checking medications labels with the doctor’s order and medication administration record sheet for correct type and amount of medication.
• Create medication training or consumer-specific training identified and provide education to care providers and individuals, if appropriate, in person or via telehealth, to also be placed in Taskmaster Pro and assigned to all staff caring for the individual.
• Communicate with the Pharmacy or Physician’s office on the individual’s medical needs.
• Administer and monitor the first dose of medication the individual has never received before during working hours.
• Participate in on-site clinics.
• Participate in virtual physician appointments as-needed.
• All nursing tasks completed must be documented in Taskmaster Pro.
• All time spent on non-nursing tasks must be documented and submitted for review.
• Upon completion of Focused Assessments, a handoff report will be performed to the Lead RN.
• Must operate within the LVN Scope of Practice, Lead RN must be kept up to date on the health status and changes of all assigned Mary Lee individuals.
• In an emergency, the Clinical Supervisor, Lead RN, must be notified by the LVN to ensure assistance is available since the situation is no longer stable and predictable.

Role Requirements:

• Documentation of ALL nursing tasks performed must be entered into EHR within 14 calendar days as stated in state guidelines. The nursing task documentation requirement goal per week for this role is 50%.By signing this job description, you are agreeing to meet this requirement.The HCS billing guidelines will be attached for your reference.
• Documentation of ALL time spent on non-nursing tasks must be documented and turned in for review every payroll.
• Total sum of nursing task documentation and non-nursing task documentation submitted must equal the amount of hours the employee was ‘clocked in’ for during the 40 hour work-week.
• PRIOR to working overtime, approval is required in writing fromsupervisor.
• Attending regularly scheduled meetings in person or via telehealth with the Nursing Team to ensure communication, consistency, and support are present.
• Communication and professionalism with all coworkers to maintain a positive work environment.
• Work as a part of a team to provide nursing care to the individuals at Mary Lee.

Qualifications:

• Advanced understanding of basic office applications, including MS Office (Excel, PowerPoint, Outlook)
• Experience in or ability to learn Electronic Health Records
• Excellent interpersonal skills
• Excellent written and verbal communication skills
• Excellent organizational and time management skills
• Aptitude for Problem-solving
• Ability to travel a reasonable distance as required to ensure quality
• Valid LVN License to practice in Texas with at least 2 years of experience
• Current CPR Certification
• Experience in the HCS/ICF program working with people with disabilities
• Must pass Criminal Background Check
• Must hold Valid Texas Driver’s License and have reliable transportation
