Mary Immaculate Health/Care Services is Hiring a Wellness Nurse - Weekends Near Lawrence, MA

16 hours/week every Saturday and Sunday 7a-3:30p or 8a-4:30p (flexible) at Marguerites House Assisted Living Community

Position Summary:

Responsible for the assessment of the resident, design of the service plan, and implementation and evaluation of the plan. Provides orientation and supervision to the Certified Nursing Assistants/Resident Care Assistants. Assists with the care management of residents and communicates as necessary with caregivers and other ancillary service providers.

Responsibilities includes the following, but not limited to:

  • Provide supervision, scheduling, hiring and training of all Certified Nursing Assistants/Resident Care Assistants and coordination of private home healthcare personnel.
  • Work collaboratively with social workers to efficiently coordinate admissions process for residents, performing assessments and completing service plans as necessary.
  • Perform as a member of the wellness team to provide overall care management of the residents.
  • Communicate with residents, families, providers and staff as necessary to assure delivery of care and service.
  • Assess, design and implement service plans for residents under the direction of the Wellness Director and Service Coordinator.
  • Contribute to, plan and attend all educational sessions for wellness staff in keeping with requirements of the Executive Office of Elder Affairs and Marguerite's House policy.
  • Operates within budget for department as instructed by Director and Administrator.
  • Completes clear and accurate documentation as necessary to assure delivery of care and compliance with Executive Office of Elder Affairs, Marguerite's House and GAFC program requirements.
  • Implements and evaluates, with Director all contracts with outside health vendors to meet the needs of residents.
  • In collaboration with the Director, develops, implements and evaluates all healthcare policies and procedures of the wellness management of the Assisted Living residence.
  • Performs all duties in a manner consistent and reflective of the mission of Marguerite's House.
  • Maintains confidentiality regarding the residents and operations of Marguerite's House.
  • On-call responsibilities for issues relating to residents health and wellness, staffing or general emergency (rotation – includes some weekends).
  • Understands and utilizes all emergency response systems within the residence and follows all procedures.
  • Reports all incidents to supervisor or charge person for the shift as they occur and completes all necessary documentation completely and accurately.
  • Demonstrated knowledge of MDS Sheets book.
  • Serves as member of committees as necessary.
  • Abide by the Standards of Conduct which is the basis of the Organizational Integrity Program.
  • Compliance with Health Information Privacy (HIPPA) related laws and regulations.

Qualifications

Licensed Practical Nurse or Registered Nurse in Massachusetts. Three years of experience in the delivery of care of older adults. Experience in home health care, an assisted living residence, housing, or continuing care retirement community preferred.
