Marty's Orchid House is a licensed day program that services young children from the ages of 18 months to 6 years that have sensory processing disorders and overlapping health conditions. A child's day is filled with a structured classroom schedule at a 3:1 child-to-caregiver ratio. The RN Care Coordinator (RNCC) should be highly motivated, passionate and innovative, providing comprehensive services in the day center setting. The main role of the RNCC will be to assess and evaluate children and to implement his/her plan of care each day. The RNCC should take a holistic approach to care, as the entire child, their environment and the thoughts and opinions of their caregivers/parents are taken into consideration when implementing treatments/interventions. The RNCC should have excellent communication skills, as carryover plays an integral part in meeting the child’s needs.
Job Description
The RNCC shall maintain compliance with the standards of nursing practice and delegation and shall: participate in a child’s pre-admission planning conference, assess patient health problems and needs, develop and implement nursing care plans, provide hands-on nursing care, maintain medical records, participate in multidisciplinary team meetings for a child, provide care coordination for a child, and provide education and training to a child’s parent or guardian on a variety of health-related topics. Kentucky licensing or registration required.
Although not all RNCC’s may be assigned to every task listed below, the following is a comprehensive list of tasks that may be assigned regularly, or as needed:
Role and Responsibilities:
Report a child’s changing condition, or any incident that has the potential to cause injury, immediately to Management and EDON. Notify parent/guardian as soon as the situation is stable and under control.
Obtain relevant health records following health appointments, surgeries, and hospitalizations to maintain a comprehensive health record for assigned children.
Oversee Orchid House Technicians (caregivers) and other subordinate staff providing direct care to a child; delegate tasks as needed to achieve goals outlined in assigned children’s CCP.
Make referrals to appropriate resources including community, social, educational, and financial services.
Maintain collaborative, constructive, and frequent communication with assigned children’s parent/guardian about a his/her progress, goals, and specific areas identified as at-risk for future problems; provide parent/guardian education as needed.
Communicate effectively with managements, co-workers, and subordinates.
Assist with meals and center cleaning tasks
Assist in assigned classrooms during peak times and during challenging times when staff may need assistance.

Requirements:
Minimum of Associate's Degree in Nursing
1 years pediatric and/or behavioral health experience preferred
