Licensed Nurse- Assisted Living

Bay Vista Commons

Bremerton, Washington

*New Wage Scale*

Starting wage is $29.00/hr. up to $38.34/hr. depending credentials and related experience as determined by Supervisor and HR

Bay Vista Commons is seeking an experienced Licensed Practical Nurse or a Registered Nurse with a passion for senior care to join our nursing team. The Licensed Nurse will work closely with the Director of Nursing (DNS), ADNS, Assisted Living & Memory Care Coordinators, etc., to support resident care, staffing and assist with staff training, compliance and quality measures.

Typical Duties:

  • Participate in admission assessments
  • Participate in 14 and 30 day new resident assessments/care plan adjustments
  • Communicate and coordinate with physicians as needed for resident care/resident orders
  • Communicate with DSHS Social Workers
  • Communicate all aspects of resident care with DNS and ADNS.
  • Assist with investigations on all fall and incident reports
  • Notify State Hotline with any resident-to-resident incidents or any broken bones from falls.
  • Assist Staff with resident notifications, admissions, transfers, and discharges/deaths.
  • Participate in a regular staffing rotation and assist with PRN coverage.
  • Demonstrate and model clinical skills, critical thinking and nursing interventions.
  • Assist with regular Medication Cart audits, monitoring and obtaining medications from pharmacies.
  • Provide ECP reports to DNS/ADNS with clinical input/insight.
  • Develop and implement processes to monitor resident wellness, regular provider visits, dental visits, etc
  • Regular resident interactions including room safety checks, housekeeping checks, resident change monitoring.
  • Ensure that all BVC policies and procedures are followed.
  • Ensure that all infection control procedures are carefully followed
  • Notify DNS, ADNS and administrator when a resident is sent to the hospital, ER or admitted to Hospice.
  • Complete 2 Step PPD Tests for all new employees.
