Marin Community Clinics is Hiring an ASW or LCSW (Pediatric Complex Care Provider) Near Novato, CA

Overview

Marin Community Clinics is looking for a mission-driven Associate Social Worker (ASW) or Licensed Clinical Social Worker (LCSW) to join our dynamic, multidisciplinary, Complex Care team to advance our goal of improving health outcomes for children with complex medical, behavioral health, and psychosocial needs. Our Complex Care team is committed to increasing health equity among Marin’s most vulnerable patients through person-centered, trauma-informed, BH interventions and case management supports.

To this end, The Pediatric Complex Care Provider will offer Healthy Steps BH interventions and/or comprehensive case management support to a panel of children and families with the goals of minimizing barriers to care, improving health outcomes, and helping families increase capacity to manage their needs with greater independence. The Pediatric Complex Care Provider will work collaboratively with Pediatrics, Behavioral Health, and other members of the clinical care team and will report to the Director of Complex Care.

We are looking for a Complex Care Provider who:

  • Understands how ACES/trauma may impact childhood development.
  • Understands the interplay between chronic conditions and psychosocial issues. 
  • Demonstrates cultural humility when working with families and children.
  • Demonstrates compassion for and understanding of issues related to homelessness.
  • Has passion for helping individuals navigate a complex system of care.
  • Is a collaborative team player.
  • Demonstrates a willingness to be flexible and adaptable in all aspects of this work.

Responsibilities

  • Provide Healthy Steps supports/interventions to vulnerable families with a focus on parenting, attachment, and development;
  • Provide dyadic, individual and/or family therapy for Healthy Steps Families;
  • Provide intensive case management services to a small caseload children/families ;
  • Contribute to program development and expansion of the Healthy Steps program across clinics
  • Track referrals and provide follow-up to ensure service delivery
  • Timely and accurate reporting of data as requested to MCC CM programs
  • May travel between clinics or meet Patients in the community as needed
  • Provide frank and considered opinions regarding dyadic/case management services and quality improvement measures
  • Other duties may be assigned.

Qualifications

Education and Experience:

  • Associate Social Worker California or License Clinical Social Worker in California is required.
  • 1-2 years’ experience (and passion for) providing support to children and families with chronic and complex BH needs, complex medical conditions and/or substance-using individuals highly preferable;
  • Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required;
  • Electronic Medical Records, EPIC experience desired but not required.

Required Skills and Abilities:

  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence in English.
  • Bilingual skills - Must be able to communicate with patients and partners in Spanish.
  • Effectively present information and respond to questions from clients, customers, and the general public in Spanish.
  • Strong preference for a candidate that is able to conduct clinical assessment interviews and case management interventions with patients in Spanish.

Computer Skills: To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.

Certificates, Licenses, Registrations:

  • CPR Training
  • Valid CA Driver's License

Physical Requirements:

  • Prolonged standing, walking, and prolonged periods sitting at desk and working on a computer.
  • Must be able to lift up to 15 pounds at times when necessary.
  • Fine motor hand movements typical of performing standard ASW duties.
