Salary: 90,000 - 140,000

Summary:

The Marin City Health and Wellness Center MCHWC Family Nurse Practitioner/Physician Assistant works collaboratively with the Chief Medical Officer to provide a wide range of primary care services which includes health promotion, injury and illness prevention, diagnosis, treatment, and monitoring of commonly encountered health problems and injuries. As a part of the MCHWC key staff, the mid-level practitioner works with the MCHWC’s Chief Medical Officer to serve as a community liaison and advocate. The Practitioner upholds the standards and guidelines of the MCHWC and provides healthcare services in a holistic, patient care-centered framework.

Essential Duties and Responsibilities:

  • Assesses the health status of patients by reviewing medical histories, initiating, performing screening and diagnostic tests, and other procedures appropriate for the patient’s condition.
  • Assist the CMO in medical procedures.
  • Prepare care plans for patients and monitor the conditions of the patients.
  • Interpret and evaluate individuals' health care requirements.
  • Write prescriptions.
  • Communicate with patients closely and emphasize disease prevention and wellness strategies.
  • Contribute to the success of the Center through quality improvement initiatives.
  • Maintain professional working relationships with all staff, clients and the public.
  • Knowledgeable of policies and procedures for safe work practices.
  • Demonstrate knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care.
  • Consult with CMO as dictated by patient need and assessment findings.
  • Establish and maintain communication with other MCHWC providers, staff, and volunteers to promote consistency and continuity of care.

Qualification Requirements:

Education and/or Experience:

  • Current license to practice as a Nurse Practitioner or Physician Assistant in California
  • Master’s Degree in Nursing and National Certification from accredited schools
  • Experience working in an outpatient health clinic setting. Experience in a community health center setting working with underserved populations preferred.
  • Valid DEA Certificate
  • Current CPR (BLS) certifications.
  • Experience working in an outpatient health clinic or office setting. Experience in a community health center setting
  • Experience with administration of community health programs, and staff supervision
  • Demonstrated experience with Electronic Medical Records.
  • Mission-driven and passionate about community health issues.

Language Skill:

  • Excellent customer service and communication skills
  • Strong verbal, written, and interpersonal skills

Reasoning ability: 

  • Ability to recognize problems, collect data, and establish facts
  • Ability to understand and apply guidelines, policies and procedures

Equipment/Machinery: Fax machine, copier, personal computer, telephone, calculator, Microsoft Word/Excel, EHR system and other software as required.

  1. Stand and walk or sit alternatively depending on specific needs of day. Estimate 30% of time is spent on feet and 70% sitting at desk.
  2. Have occasional need to perform the following physical activities: bend/stoop/squat, climb stairs, push or pull, reach above shoulders.
  3. Have occasional need to perform standing and walking activities.
  4. Constant need to perform the following physical activities: writing/typing, grasping/turning, finger dexterity.
  5. Lifting/carrying over 10 pounds occasionally. Lifting/carrying less than 10 pounds frequently.
  6. Vision requirements: constant need to complete forms, read reports, view computer screen. Frequent need to see small detail. Frequent need to see things clearly beyond arm's reach.
  7. Hearing requirements: constant need to communicate over telephone and in person.

TRAVEL REQUIREMENTS: Must be willing to travel locally between Clinic locations. May be an occasional need to travel for training or events.
