We Require
One year of experience providing behavioral health care and services
Bachelor's Degree in Social Work, Psychology, Sociology, or a related field
We Value
Demonstrated case management and discharge planning experience
Experience working with RBHA services and criminal justice processes
Intermediate proficiency in building Excel reports to coordinate and track discharge status
Experience working with an electronic health record
Bilingual skills in English and Spanish
Job Contributions
Responsible for facilitating with outside providers, vendors, and stakeholders for the discharge planning of inmates. Ensure SMI and others at risk due to medical, mental health, or substance use problems will receive a warm handoff at release from jail to their assigned provider or Mercy Care Arizona provider
Develop and coordinate community re-entry plans that address risk factors associated with recidivism in the criminal justice system
Coordinate with a multidisciplinary team regarding patient care to develop patient discharge plans, including community entities
Assist qualifying patients in AHCCCS enrollment
Obtain treatment records and coordinate care with family, community providers, and others relevant to the patient's care and community re-entry plan
Maintain and update a list of community resources to support the patient's transition back to the community
Maintain HIPAA patient confidentiality in the management of information
Comply with the county policies and procedures related to the safekeeping of Protected Health Information
Adhere to all applicable professional standards and requirements, including modeling the code of ethics associated with licensure and certification and meeting regulatory, licensure, accreditation, health assessment, and CPR requirements
Working Conditions
Must be able to work inside a jail environment, working close to inmates and other staff, and being indoors in a confined area handling multiple or complicated tasks
Requires accuracy, decision-making, and concentration skills to be vigilant in a teamwork environment
Performs a high volume of work under short deadlines or with time pressures to complete frequently changing and unscheduled tasks
Complete Health-e-Arizona Plus (HEA ) training within the first three months of hire to become a certified correctional assistant for AHCCCS applications
Must possess or have the ability to obtain a valid Arizona driver's license by the time of hire
