As the Community Health Assessment Coordinator, you will facilitate and support Maricopa County's Community Health Needs Assessment (CHNA) for Maricopa County, and its collective (Synapse) made up of non-profit hospitals and Federally Qualified Healthcare Centers (FQHC). This work is done through active facilitation and collaboration of community and healthcare partners, data gathering and analysis, research, report writing, and presentation delivery for our public health department and healthcare partners. This position works in the Health Outreach, Partnerships, and Equity (HOPE) Division.

We Require
One year of experience supporting and coordinating public health community engagement programs or similar large-scale community-based project management requiring data collection and analysis, creating protocols, and monitoring deliverables
Bachelor's degree in Public Health, Community Health, or a closely related field
A combination of education and experience may be considered in substitution for the minimum qualifications
We Also Value
Knowledge of public health frameworks for community planning, social determinants of health, health equity, and community health needs assessments
Proficiency in Microsoft Word, Excel, and PowerPoint
Two or more years of experience working with community partners
Two or more years of experience leading or supporting multi-team collaborations
Job Contributions
Plan, organize, coordinate, and administer HOPE-related CHNA outreach deliverables, ensuring alignment with MCDPH, PHAB, and MAPP 2.0 requirements
Establish CHNA outreach goals, evaluate and determine program effectiveness, and measure and report on program success to mobilize community outreach efforts
Conduct community outreach and foster partnerships to encourage program participation, such as leading event registration, staffing, training, and liaising with external contracted and/or volunteer partners supporting CHNA efforts
Monitor grants and program contracts; participate in developing the program budget and support adherence to budget, in particular relating to program inventory and equipment, outreach material development, translation, and printing
Conduct program-related studies and prepare written CHNA reports, recommendations, and presentations for external partners
Coordinate developing training and informational materials to promote public understanding of program activities, services, and requirements for CHNA event outreach specialists
Work with other departments and outside agencies to coordinate and develop tools and resources for CHNA data collection efforts; Support Synapse coalition as needed
Formulate policies and procedures; implement and make changes as appropriate. Utilize existing protocols and provide feedback, evaluation, and documentation on opportunities for process improvement
Working Conditions
Work is primarily performed in a business office setting
Must possess or have the ability to obtain a valid Arizona driver's license by the time of hire
Occasional travel using a personal or county-issued vehicle may be required to attend meetings with program participants and community partners
May be required to attend work-related events and perform community outreach activities outside of normal working hours
Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test
