PACT Tenancy Support Specialist
Full Time/ Non-Exempt/ Helena

Position Overview:
Under the supervision of the PACT Team Lead, the Specialist performs duties responsible for addressing the housing needs of adults who are chronically homeless and disabled, seeking permanent housing. This person will be responsible for the day to day operation of the housing component for the PACT team, including program compliance with housing files.
Job Duties:

1. Provides street outreach to individuals in need of behavioral health care with the goal of connecting them to a more comprehensive integrated health care system.

2. Coordinates with partners of Housing First, HUD Programs, and other identified community housing agencies. to identify and connect clients with appropriate behavioral health care and work to support the individual in housing through the partners.

3. Responsible to assess, identify, arrange for, or provide pre-tenancy, housing stabilization and tenancy. sustaining, and housing focused service coordination in a team concept.

4. Assists clients in accessing basic needs directly related to their behavioral health care needs such as shelter, food, obtaining prescriptions, etc. as part of the PACT team.

5. Identifies affordable housing units, develops, and maintains positive relationships with local property owners and landlords. Assist clients with placement in transitional housing as appropriate. Provides advocacy from a client’s perspective in program decision making and planning as it relates to housing.

6. Attends and participates in housing meetings in the community Provide Housing Retention Counseling to clients of the program.

7. Completes housing packets for clients receiving vouchers through the coordinated entry or other housing opportunities.

8. In coordination with PACT, provides training on housing retention and landlord recruitment to community agencies.

9. Guides clients through process of housing orientation and lease up in coordination with agency responsible for issuing voucher. Assists with locating bridge housing and gathering documentation necessary to issue housing voucher.

10. Meets with clients to review site rental options.

11. Serves as liaison and helps resolve problems between various parties including clients, landlord/property owners/property managers, rental assistance programs, Housing First staff, housing agencies, and agency providing ongoing supportive services.

12. Provides relocation assistance to clients as needed (identify new unit, complete all necessary forms for relocation).

13. Networks with community agencies, especially homeless service, and employment service providers, for the purpose of information, coordination, and/or development of services. This may include attending community meetings, committee work, and/or presentations to interested agencies.

14. Reads, understands, and interprets HUD Housing regulations for various programs, specifically Project based vouchers, HUD Program tenant-based vouchers.

15. Conducts and/or assists with housing searches, housing orientations, HQS inspections/lease ups, re-certifications, and move-out inspections (as appropriate) for the program.

16. Maintains housing database for the program.

17. Provides housing consultation to employees regarding clients.

18. Outreaches and engages program participants who are chronically homeless and disabled.

19. Maintain and update client information in LWSI.

20. Attend and actively participate in Team meetings and supervisory conferences.

21. Maintains assigned caseload. Caseload depends on the client and community needs for the PACT Team concept. Caseloads will be assigned by the Team Lead.

22. Observe established administrative procedures.

23. Performs medication delivery, works rotating weekday and weekends, and must pass MRWH medication certification.

24. Provide written reports and maintain statistics as required.

25. Attend and participate in employee training opportunities.

26. Adhere to MRWH policies and procedures including client confidentiality provisions.

Qualifications:

  • Must have a Bachelor’s degree in human services field, plus as least one year of full-time experience serving people with mental illnesses or individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary knowledge and skills. OR
  • Graduation from high school or GED and a combination of relevant experience and/or education equaling at least four years.)
  • Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in their community. Must possess the skills and competence to establish relationships with persons with severe and persistent mental illnesses.
  • Respect for client rights and personal preferences in treatment are essential.
  • Must have a valid MT Driver’s License, vehicle insurance, and vehicle.
  • Transportation of clients required.


$2,000 Sign on Bonus!!
