MaineHealth is Hiring a Remote Clinical Pharmacist – Primary Care - Remote Patient Management Program

Summary

  • Position Summary:

    Within MaineHealth’s primary care pharmacy program, this clinical pharmacist will be a part of our growing primary care pharmacy team. In this role, the pharmacist will help provide seamless medication access and medication management for patients with hypertension enrolled in our Remote Patient Monitoring Program along with our program Health Coaches. This clinical pharmacist position will be focused on providing high quality patient care and education remotely through virtual care technologies. The clinical pharmacist will collaborate closely with MaineHealth Telehealth Services, our Population Health team and our Primary Care Pharmacists throughout MaineHealth. Within the role, the clinical pharmacist will be included in operational workgroups to provide input on process development, implementation and program sustainability. Initial clinical focus will be hypertension management, additional chronic diseases, including but not limited to diabetes and hyperlipidemia, may be managed by this pharmacist as the program develops.

    Some core functions include:

  • Work collaboratively with all healthcare professionals to provide safe and cost-effective patient care.
  • Communicate directly with patients and their families on all aspects related to medication therapy promoting high levels of knowledge, engagement, and adherence.
  • Ensure appropriate and optimized medication therapy for patients with chronic disease including, but not limited to, hypertension.
  • Provide education to other healthcare professionals including precepting trainees to enhance clinical competency on topics related to medication therapy.
  • Partner with other healthcare professionals to develop evidence-based clinical pathways.
  • Generate and analyze process and outcome measures for use in assessing medication-related patient outcomes.
  • Demonstrate ongoing professional development including meeting continuing education requirements, keeping abreast of current professional literature and how it relates to clinical, financial and safety outcomes.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Pharm.D. or BS. Pharmacy from an accredited College of Pharmacy and eligible for pharmacist licensure through the Board of Pharmacy in the State of Maine.
    • PGY1 Pharmacy residency strongly preferred.
    • Ambulatory care experience with direct patient care strongly preferred. Strong interest in drug therapy management for patients with chronic diseases required
