What you will do:

• Coach and mentor individuals to meet their personal goals.
• Assist with daily living skills and support overall health and wellness
• Support treatment plan goals.

Responsibilities

• Follow individual client treatment plans.
• Collaborate with staff and family to achieve desired treatment outcomes.
• Document the services provided at each shift
• Track and attend medical/mental health appointments.

Required Minimum Knowledge, Skills, and Abilities (KSAs) – “What you will need”:

• Education: Bachelor in social work or related field preferred
• License/Certifications: None required
• Experience: Minimum of two years’ experience working with behavioral health consumers and families. Experience working in team based settings and collaborating with multiple providers preferred.
