Job Overview:

We are seeking a dedicated and compassionate Registered Nurse (RN) with expertise in wound care to join our home health team. As a Home Health Wound Care RN, you will be responsible for providing specialized wound care services to patients in the comfort of their own homes. You will assess, monitor, and treat various types of wounds, collaborate with the interdisciplinary team to develop and implement comprehensive care plans, and educate patients and their families on wound management techniques and preventive measures.

Responsibilities:

• Conduct thorough wound assessments and document findings accurately
• Develop individualized wound care plans based on assessment data and evidence-based practices
• Provide wound care treatments, including cleaning, dressing changes, and wound debridement
• Monitor wound healing progress and make appropriate adjustments to treatment plans as needed
• Educate patients and caregivers on proper wound care techniques, infection prevention, and signs of complications
• Collaborate with physicians, physical therapists, and other healthcare professionals to ensure coordinated care delivery
• Maintain accurate and up-to-date patient records and documentation in compliance with regulatory standards and agency policies
• Participate in ongoing education and training to stay current on best practices and advancements in wound care management

Qualifications:

• Valid Registered Nurse (RN) license in the state of Washington
• Minimum of 1 years of clinical experience as an RN, with a focus on wound care preferred
• Certification in wound care (e.g., Wound Care Certification Board, Wound Ostomy Continence Nursing Certification Board) highly desirable
• Ability to work independently and collaboratively in a multidisciplinary team environment
• Current Washington state Driver’s license with proof of insurability.
• Reliable transportation
