Chronic Care Coordinator provides care for all patients and specifically all patients within the Madisonhealth Chronic Care Program. S/he develop, monitor, and evaluate care specific of the age of the patient population served in the medical clinic. S/he participates as an active team member in the quality programs that physician clinics are involved in. S/he can perform general nursing duties and participates in performance improvement activities. S/he is expected to demonstrate quality and effectiveness in work habits and clinical practice and treat staff, physicians, patients, and families with consideration and respect.

Schedule: Full time

• Monday-Friday 8am-5pm
