Madison Health, through our partnership with New Vision, is looking for an Intake Coordinator for our medical withdrawal management program. New Vision is a medically supervised, hospital-based service for adults experiencing impending or active withdrawal from alcohol, prescription medications and/or certain drugs. New Vision provides a safe, confidential, compassionate and reliable care for people who qualify.
About the position: As a New Vision Intake Coordinator, you will be responsible for performing a variety of social service duties as explained in the Job Description and as may be assigned by supervisor. The position is responsible for managing telephone inquiries concerning the New Vision™ service, screens potential patients, assists them with their questions and concerns, and initiates discharge planning.

Key job qualities/duties:

• Schedule appointments for patient intake. Complete patient assessments including communication with physician and nursing staff as it pertains to continued stay.
• Rounds in patient daily.
• Completes discharge plan with patient's involvement. Conducts follow up calls on patients.
• Performs community outreach.

What qualifications are we looking for?

• Bachelor's degree in Human Services, Counseling, Substance Abuse, Criminal Justice or a related field preferred.
• Minimum of at least two (2) years of social service and/or clinical service work.
• Basic understanding of chemical dependency.
