MACT HEALTH BOARD is Hiring a Behavioral Health Case Manager Near San Andreas, CA

Position Title: Behavioral Health Case Manager

Position Location: San Andreas, California

Status: Non-Exempt

Supervisor: BH Director

POSITION SUMMARY:

Under the direction of the Behavioral Health Director, the Behavioral Health Case Manager will provide case and care coordination for MACT patients. Will assist the patients with collaborative care, referrals, navigating various systems of care to assist MACT patients with obtaining overall wellness. Will be working collaboratively with behavioral health and medical providers and patients with completing documents, maintaining scheduled appointments, assessing for safety, goal setting, planning and achievement. 

 The position requires an awareness and keen appreciation of American Indian traditions, customs, and socioeconomic needs and the ability at all times to meet and deal effectively in contacts with Indian health organizations which requires tact, courtesy, discretion, resourcefulness and good judgment in handling functions of a sensitive nature.

 PRIMARY RESPONSIBILITIES:

1. The responsibilities of a case manager will be to work collaboratively with a team of behavioral health and medical providers to assist patients with resource connection.

2. Case managers will work directly with patients, assess for safety and coordination of care. 

MINIMUM REQUIREMENTS:

1. Above average communication skills, computer skills, knowledge of the area and resources available. 

2. Preferred bachelor’s level education in psychology, social work, sociology, or a like field. 

3. Must have a high school diploma or equivalent and,

4. Minimum of 1 years’ experience in clinical setting. 

MANDATORY REQUIREMENTS:

1. Must pass Occupational Health Screening.

2. Must possess a valid form of identification as listed on the U.S. Department of Justice I-9 form, verified using the E Verify system.

3. Clean DMV driving record and MACT auto insurance driving eligibility (determined by auto insurance carrier).

4. Proficient in problem resolution, detail analysis, and accuracy.

5. Ability to work as part of a professional team.

6. Ability to work independently and submit weekly reports of progress to direct supervisor.

7. Ability to prioritize tasks and complete assignments in a timely manner.

 DESIRED ABILITIES:

1. Applicant must have exceptional communication skills, be willing to travel within the MACT communities.

2. Applicant must be comfortable working with individuals with behavioral health needs, substance use disorders and medical/ co morbid disorders. 

3. Applicant must be able to collaborate with behavioral health clinicians as well as medical and administrative staff to assist with linkage to resources. 

4. Applicant must have knowledge of community-based resources.
