Description

GENERAL DESCRIPTION

The Community Employment Case Manager shall coordinate the provision of Community Employment Services: provide case management, vocational evaluation and job training to persons served. The case manager also authors and/or directs the implementation of persons’ vocational plans through monitoring of the methods employed coordination of services and documentation of results. The case manager serves as a member of the interdisciplinary team (IDT) in planning individualized programs and evaluating service effectiveness. The case manager is also responsible for maintaining case records according to licensure/ certification/accreditation standards and Macon Resources, Inc. (MRI) policies and procedures.

ESSENTIAL JOB RESPONSIBILITIES

  • As a member of the IDT, the Community Employment case manager provides basic training/job coaching to persons at their work site as required or requested following the recommendations of the IDT and/or the employer while serving as a role model and encouraging independence and self-directed decision making skills; observes all local, state, and federal laws, licensure/certification/accreditation standards and agency policies and procedures. This responsibility requires the use of a phone, computer and vehicle.
  • Completes job/task analysis of potential job responsibilities, including methods, equipment and accessibility issues to determine an appropriate job match and to maximize individual job performance, satisfaction and job retention.
  • Monitors adherence to DHS/DRS contractual agreements for Community Employment Services.
  • Monitors the provision of training/evaluation of persons served while supporting the progression of work skills and (re)habilitation efforts in the person's work setting or community placement.
  • Market employment services and activities to community employers for all placement services (Supported Employment Program (SEP), On-the-Job Eval (OJE), Placement, job coaching, job modification/accommodation, etc.), to promote employment opportunities for persons with disabilities.
  • Plans, develops, coordinates and manages services which meet applicable local, state, and federal laws; licensure, certification, and accreditation regulations and meets the needs of individuals served. This responsibility requires the use of a phone, computer, word processing software, E-mail and the filing system.
  • Provide information/direction to employers in identification, modification, and/or removal of architectural, attitudinal and/or procedural barriers to the employment and advancement of persons with disabilities.
  • Obtains data/information pertaining to each individual's written Individual Vocational Plan (IVP), completes all documentation including; IVP, program evaluation, chronological notes, Accident/Incident Reports, intake documentation, disenrollment documentation, etc., according to MRI policies and procedures and established time frames.
  • Communicates information verbally and in writing as required and/or requested to community employers, DRS counselors, residential facilities, guardians, family members, and/or supervisor in a timely manner.
  • Provide individual/group instruction to persons on job seeking skills, job-retention skills, and/or other skills identified as barriers to obtaining and retaining employment.
  • Completes program evaluation forms and submits to the supervisor according to established time frames.
  • Monthly re-evaluates objectives, training methods and provides data/information to the primary case manager for inclusion in monthly reviews and IPP according to established time frames.
  • Serves as a member of the IDT, represents Community Employment Services in needs identification, program planning and development. Conducts and/or obtains formal/informal assessments according to IDT recommendations in order to identify individuals’ needs, recommend services, and evaluate effectiveness of the IPP/IEP/IVP. Monitors the implementation of the IPP/EIP/IVP.
  • Provides linkage/referrals according to established policies and procedures to internal and external services as necessary to meet identified needs and preferences.
  • Maintains and updates master case records for persons served in the Community Employment program according to MRI policies and procedures and all program standards.
  • Intervenes in crisis situations as necessary to prevent physical or emotional injury to individuals served or others.
  • Drives safely in an official capacity to conduct agency business for MRI and on occasion may provide transportation to individuals/families served; observes all local, state, and federal laws, as well as all agency policies and procedures.
  • Responsible for care of equipment and training materials; informs supervisor of missing and/or damaged materials; requests additional equipment and materials as needed.
  • Assumes the responsibility to attend and complete the initial MRI training requirements (core training and job-specific training). On an ongoing basis, attends and participates in any required recertification or refresher sessions and/or other training deemed appropriate by the supervisor and/or the Program/Department Director within specified time frames.
  • Participates in agency in-service training sessions as assigned by the supervisor and/or Program/Department Director and/or on a voluntary basis to enhance one's knowledge and skills.
  • Administers standard first aid and CPR procedures on an as-needed basis, while maintaining current certification for both.
  • Completes individual daily payroll forms and monitors weekly efficiency report, making required corrections, and submits all documents to business office according to established time frames.
  • Completes and monitors the completion of basic housekeeping and sanitation on a daily basis for the work area:
    • Reports housekeeping needs (supplies, cleaning, etc.) to immediate supervisor on a daily basis.
    • Assures that adequate supplies are available in assigned areas to meet the hygiene needs of the individuals served.
  • Communicates, retains, and releases information (both written and verbal) on a need-to-know basis and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and Macon Resources, Inc. Policies and Procedures.
  • Practices and enforces MRI Safety Policies and Procedures and assures the safety of employee, persons/families served, agency guests, and agency property.
  • Keeps supervisor informed verbally and in writing of problems and activities within assigned area of responsibility; refers matters beyond the limit of authority and expertise to the supervisor for direction.
  • Performs special projects and other related work as required or requested within scope of job responsibilities.

Qualifications


JOB QUALIFICATIONS

  • Bachelor's degree required. Prefer degree in human services related field.
  • Prefer one or more years of previous experience working with individuals with developmental disabilities, mental illness, and/or physical disabilities; experience in job development or public contact.
  • Must have knowledge of developmental disabilities, mental illness, active treatment, principles of normalization and least restrictive environment, choice, integration, and/or demonstrate the willingness to learn.
  • Must be able to adapt to frequent change and performs adequately when confronted with critical or unexpected issues; make sound clinical judgments based on sound reasoning in daily and emergency situations.
  • Must be able to communicate effectively and diplomatically in both oral and written form and be able to meet and work with the public.
  • Must be able to adequately use 100 basic manual communication signs and/or complete sign training according to established time frames in job specific training.
  • Must be able to meet the following physical capabilities:
  • Sit and/or stand for prolonged periods of time.
  • Bend, twist, stoop or kneel.
  • Vision correctable to 20/20 or sufficient to complete job responsibilities.
  • Exhibit manual dexterity.
  • Adequate hearing.
  • Must be able to counsel, advise, teach, resolve conflicts, and direct employee and families.
  • Must be able to provide quality supervision to paraprofessional and professional employee.
  • Must be able to adhere to the Safety Policies and Procedures of Macon Resources, Inc.
  • Must be able to maintain payroll, financial, personnel, and information related to persons served as confidential and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and the Policies and Procedures of Macon Resources, Inc.
  • Must be able to complete MRI training requirements.
  • Must have reliable transportation to conduct agency business and/or to transport individuals served as needed; must maintain a valid Illinois Drivers License and a good Motor Vehicle Record; and must maintain personal auto insurance with minimum coverage of Bodily Injury-Liability per Person of $100,000 and Liability per Accident of $300,000, Property Liability of $100,000; Medical payments of $5,000; and Uninsured/Underinsured Personof $100,000 and Accident of $300,000.
  • Must have or be able to obtain first aid and CPR certification within 60 days of employment and maintain certification throughout employment.
  • Must be able to utilize a phone, computer, wordprocessing software and E-Mail and the filing system.
  • Must be able to work a flexible schedule including evening and weekends.
  • Must be able to summarize, synthesize and analyze data/information.
  • Must demonstrate familiarity with local service systems and service provider agencies.
  • Must be able to complete job responsibilities independently and have problem-solving abilities with minimal supervision.
