Lyndon Institute is a comprehensive, independent high school providing thoughtful and rigorous preparation for students of the Northeast Kingdom of Vermont and beyond to reach their intellectual, creative, and physical potential. LI is seeking to hire a full time Health and Wellness Case Manager to work during our academic calendar. This position will work 10-months and report directly to the Director of Health and Wellness.SUMMARY: The role of the Health and Wellness Case Manager is to help students succeed academically, socially, behaviorally, and emotionally; to collaborate with educators, parents, and other professionals to create safe, healthy, and supportive learning environments that strengthen connections between home, school and the community.RESPONSIBILITIES:●Facilitate support groups for students●Schedule and facilitate home visits with students and families●Attend IEP/504/EST meetings for students●Act as a liaison for students/families with outside resources and agencies●Teach J-term classes around the topic of health and wellness●Meet individually with students and maintain a clinical caseload●Coordinate with classroom teachers/special education case managers●Support campus life office as needed●Maintain accurate records●Other duties as assignedQUALIFICATIONS:●Bachelor’s degree in human services, counseling, psychology or a related field is required. Master’s degree in counseling, psychology, social work or a related field or licensure in counseling/psychology/social work is preferred.●Demonstrated ability to be flexible and work collaboratively with faculty/staff, parents and administration.●3 years of experience working with children, adolescents and families●A career focus in human services/social work, education, therapeutic services, and/or clinical services.
