This professional social service position is assigned to the Targeted Case Management (TCM) Unit of the Lycoming-Clinton MH/ID Program. This position will function through a community-based model as a Blended Case Manager with a caseload of up to thirty (30) children, adolescents and adults who are diagnosed with chronic persistent mental illness, emotional disturbance, or the potential for developing an emotional disturbance, and are receiving or need to receive multiple services. Case management services will be available to consumers and families 24 hours per day, 7 days per week through an on-call rotation during the evening and weekend hours. This Case Manager will provide direct services to adults, children, and adolescents with expected outcomes to include a systemic focus which assures that the child will remain with his/her family, attend school, participate in local leisure and recreational opportunities, obtain employment, and maintain age-appropriate relationships with family and friends. Expected outcomes for adults will involve residing in the least restrictive community setting as possible and supporting the consumer in other goals such as employment, coordination mental health services, etc. This position will be supervised by the Targeted Case Management Supervisor.

Essential Functions

An individual must be able to successfully perform the essential functions of this position with or without a reasonable accommodation.

· Complete a strength-based assessment within one month of opening a case to assess a child’s medical, family, social, physical, and educational history, to determine what led to a TCM referral. Review and update/revise according to Community Care Behavioral Health Managed Care Organization (CCBMHCO) and Office of Mental Health and Substance Abuse Services (OMHSAS) requirements.

· Complete the Environmental Matrix with consumers and families to open the TCM case. Review and update/revise according to Community Care Behavioral Health Managed Care Organization (CCBHMCO) and Office of Mental Health and Substance Abuse Services (OMHSAS) requirements.

· Develop strength-based goal plans for every consumer and parent within one month of opening a case and Review and update/revise according to Community Care Behavioral Health Managed Care Organization (CCBMHCO) and Office of Mental Health and Substance Abuse Services (OMHSAS) requirements.

· Develop with the consumer and family a crisis plan that they will retain a copy of and be encouraged to use when necessary. Review and update/revise according to Community Care Behavioral Health Managed Care Organization (CCBMHCO) and Office of Mental Health and Substance Abuse Services (OMHSAS) requirements.

· Plan interagency services based on strengths of the child and family and include any activities which are necessary to help the child and family in the home, and to become integrated into his/her community.

· Assist the family in building a support network for the child and his/her family members.

· Utilize community resources to include assistance to children and family members in identifying and accessing needed services and entitlements. Assist in learning to access these community resources. Assist families in gaining transportation to these resources.

· Develop linkages with resources

· Maintain coordination and brokering with other child servicing systems working as a team to provide the best service to the child.

· Monitor service delivery and assure continuity of care in the mental health system and among other child serving systems.

· Implement creative strategies to assist the child and family in gaining access to resources and required services identified in the treatment or service plan.

· Advocate for the child.

· Provide services on site of their natural occurrence, such as family home, school (only when necessary), neighborhood, etc., rather than in the Targeted Case Manager’s office.

· Assist with any psychiatric placement i.e., hospitalization or residential facility placement.

· Documentation of billable contacts will be put into applicable State and/or Joinder computer systems within one week of contact.

· Attend regularly scheduled staff trainings to further develop skills.

· Participate in individual and group case management discussions to develop strategies.

· Complete the Environmental Matrix with consumers and families at least once every six months and when needs change to determine level of case management required.

· Provide on-call coverage as scheduled.

· Perform other duties as needed to complete program objectives.

· This Case Manager will be expected to manage his/her time between direct and collateral contact or travel time, and time in the office for paperwork and supervisory activities in order to meet the expectation of a monthly average of at least 334 billable units per month or 4000 units per year.

· Identify High Risk Consumers at Unit staff meetings to show effective use of common ground tools and resources to provide recovery principles.

· Participate in individual and group case management discussions to develop strategies for working with consumers, including Risk Management meetings as needed, and Community Care High Risk Management meetings on a rotating basis.

· If needed, develop a High-Risk list of consumers that may need additional staffing and development planning.

· Case Manager will respond to all calls from consumer’s providers as soon as possible and represent the Agency in a professional manner. They will accommodate the provider’s requests/recommendations in the interest of the consumer. The Case Manager will coordinate care with all community providers which the consumer is working with.

· Promote maximum consumer growth towards recovery and successful community tenure in terms of reduced hospitalizations.

· Enhance consumer’s independent living arrangement, vocational/educational activity, and natural support system.

· Assist consumers with accessing housing, income, benefits, transportation, employment services, medical services, or other non-mental health supports necessary to complete Agency goals. Assure clients coordinated access to services and provide transportation when appropriate.

· Perform visual inspection of consumer’s residence at least quarterly to assure health and safety standards are met.

· Provide resources to resolve potential crisis situations to the consumers and family members as needed.

· Identify services based on consumer’s values, personal and cultural preferences and strengths.

· Complete required financial determinations and abatements when indicated.

Physical Activities

The following physical activities are necessary to perform one or more of the essential functions of this position.

· Communicate effectively in person or by using telecommunications equipment.

· Input and gather data on a computer.

· Proofread documents accurately.

· Understand oral and written information.

· Move and relocate work related items, and if applicable, assist clients with personal items without assistance.

· Move and/or relocate child(ren) without assistance, if applicable.

· Provide transportation to consumers when appropriate.

· Enter, exit, and move about multiple residential and office settings.

· Travel to client’s homes/residences in order to serve and meet their needs in their own environments as appropriate.

· Operate standard office equipment.

This is a Civil Service position and is subject to Civil Service Rules.
