Job Description: Provide care to individuals in the comfort of their own home who have recently discharged from the hospital or skilled nursing facility. You will admit, follow, discharge and recertify patients for home health care services as well as coordinate care with physicians, clinical team and family members. Also, oversee LPN or HHA if applicable.

Job Responsibilities:

• Conducting initial nursing evaluation visit in the patients home.
• Following patient for weekly visits per patient's needs and physician orders.
• Develop, coordinate and revise the treatment plan based on physician recommendations
• Prepare accurate timely reports and required documentation.
• Adhere to all practice standards and best practices as they apply to patient care.

Job Requirements:

• Valid RN License in State of Indiana.
• IN Driver's License with the ability to drive within 15-20 miles radius daily
• Excellent charting skills (Axxess - preferred)
• Home Health experience required

Pay: $60.00 - $80.00 per hour
