Job Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to individuals re-entering from incarceration, with a focus on housing, transportation, and workforce development.

Duties:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide coaching and support services to clients
- Assist clients in accessing community resources such as Section 8 housing and job training programs
- Monitor clients' progress and adjust care plans as needed
- Collaborate with other healthcare professionals to ensure comprehensive care for clients
- Utilize motivational interviewing techniques to empower clients to make positive changes

Skills:
- Experience in case management, behavioral health, addiction counseling, social work, or related fields
- Strong communication and interpersonal skills
- Knowledge of Section 8 housing programs and workforce development initiatives
- Ability to work effectively in a fast-paced environment
