Journey Hospice in Philadelphia is seeking a Per Diem (as needed) C.N.A. to work with our hospice patients!

The Hospice C.N.A. provides personal care and light housekeeping services to terminally ill patients in the home, nursing facility, inpatient units or other environments where the Hospice patient resides in accordance with an established plan of care and under the supervision of a Registered Nurse (RN). Actively participates as a member of the Hospice Interdisciplinary Team (IDT). Provides care and comfort to patients in accordance with assigned tasks and established interdisciplinary and Hospice Aide plan of care. Observes the patient, reports and documents observations and care performed. Provides timely documentation of services based on the patients care plan

Basic Qualifications:

Valid drivers license and reliable transportation that is insured in accordance with New Jersey requirements.

High school diploma or equivalent.

Completion of an approved hospice aide training program as outlined under the Hospice Conditions of Participation 418.76: Hospice Aide/Homemaker Services; and proof of certification as a Hospice Aide from the program;

or

Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under Medicare 483.151 through 483.154, with current listing in good standing on the State nurse aide registry;

or

Completion of a state approved training program and certification as a home health aide, with proof of certification.

Must demonstrate skills and have an 80% passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 prior to working independently with a patient.
