Registered Nurse in Home Health-Full time -Missouri-Noth County area
Job Description

Summary:

Provides comprehensive nursing services to individuals and families in the community setting. Emphasis is placed on meeting the needs of persons in their homes, upon promotion and maintenance of health, and on early detection and restoration measurers. Maintains the Core Values that support the Christian Mission of Lutheran Senior Services, “Older Adults Living Life to the Fullest”.

Responsibilities

Conducts comprehensive OASIS initial nursing assessments, including physical, emotional and mental needs.

Determines eligibility for services related to homebound status and need for skilled services.

Develops an individualized plan of care in consultation with the patient’s physician and other agency personnel as appropriate.

Completes review of medications, pain management, risk status

Responsible for developing and implementing the care plan for the patient and family.

Provides intermittent skilled services, as ordered by the physician, and maintaining the highest quality of care for the patient in the home environment.

Obtains specimens as ordered by physician, submits to laboratory.

Participates in team meetings/patient care conferences, communicates with other clinicians involved in care.

Provides direct nursing care procedures to patients, Updates primary physician when necessary and at least every 60 days.

Completes OASIS at all points of care as required under PPS.

Responsible for thorough and complete documentation to ensure continuity of care, professional services and the maintenance of accurate legal documents.

Teaches family members, and/or significant others, patient care skills, accurately assess patient’s physical and emotional status, and perform services in compliance with physician orders.

Participates in on-call duties as defined in the On-Call Policy.

Ensures that arrangements for equipment and other necessary items and services are available.

Instructs, supervises, and evaluates home health aide care provided at least every two (2) weeks.

Attends in-services and department meetings as required.

Maintain CONFIDENTIALITY of all pertinent client and employee information to assure their privacy is protected.

Support and uphold all federal, state and local regulatory laws and standards.

Support and uphold all Lutheran Senior Services Policies and Procedures.

• Graduate of an accredited School of Nursing.
• Current Missouri Nursing License in good standing.
• One year home health experience preferred.
• A minimum of two years nursing experience preferred.
• Ability to provide own transportation.
• Has good clinical judgment, assessment and communication skills.
• Proficient in computer skills, to complete documentation on laptop.

The employee frequently is required to sit; reach with hands and arms; squat and stoop, kneel, crouch, or crawl.

The employee is occasionally required to climb or balance, speech clarity, speech recognition, written expression, and requires sensory abilities.

Is subject to the hazards of vehicular travel out in the community that allows for exposure to weather conditions and potential for unsafe environments.

Is subject to emotionally upset residents, or family members.

Is willing to work beyond normal working hours and on weekends and holidays when necessary.

Is subject to exposure to fumes or airborne particles, and toxic or caustic chemicals.Appropriateergonomic and personal safety equipment is provided as needed.

Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the AIDS and Hepatitis B virus within guidelines of standard precautions.
