Loyola University Medical Center is developing an innovative evidence based Transitions in Care Team.
The Transitions in Care Team (TiCT) is a newly designed enabling structure to meet the current challenges of a quaternary referral center. These challenges include: workforce availability, patient complexity, and the increasing demand for services.
The TiCT will be registered professional nurses that will function at top-of-license to coordinate and navigate transitions in care and throughput efforts (admissions, transfers, and discharges) for a wide variety of multi-disciplinary care teams. The TiCT will be integral members of the team, and in collaboration with senior leaders have opportunities to contribute to and design new models of care.
The TiCT will function Monday Through Friday during day shift hours. There will be a requirement of every 3rd Saturday - (4) 10 hour shifts

Loyola University Medical Center is a three-time ANCC Magnet © designated 547 bed quaternary care facility. The medical center is one of the most complex in the Midwest and is currently ranked 6th in the State of Illinois. The medical center is a Level I Trauma Center, designated Burn Coordinating Center, one of the largest organ transplant centers in the region, stem cell transplant center, comprehensive stroke center, and a Level III Perinatal Center by the Illinois Department of Public Health. The Candidate:
The preferred candidate will be an innovative and self-directed registered professional nurse. The ideal experience includes several years (>3) of acute care (general medical, surgical, or intermediate care) experience. It is preferred that the candidate have a broad array of complex clinical experiences, which include facilitating complex transitions in care.
