Loyal Care at Home, INC., is a Home Health agency in Van Nuys, CA. Our mission is rooted in bringing kindness and support to individuals navigating health challenges. We're seeking an experiened Quality Assurance Specialist whose primary objective will be to make corrections and enhancements to ensure that all documentation for each patient is cohesive and accurate, particularly at the start of care OASIS.

Responsibilities:

· Review incoming OASIS documents, Nursing Notes, and Plan of Care documents for accuracy, completeness, and compliance with regulatory standards.

· Identify discrepancies, inconsistencies, and errors in documentation and take appropriate corrective actions.

· Make necessary corrections and enhancements to ensure that all documentation is cohesive and accurate, particularly at the start of care OASIS.

· Collaborate with clinical staff and other departments to address documentation-related issues and ensure timely resolution.

· Provide feedback and guidance to staff members on documentation best practices and compliance requirements.

· Stay informed about changes in regulatory requirements and industry best practices related to home health documentation.

· Maintain detailed records of documentation reviews and corrective actions taken.

· Participate in quality improvement initiatives and projects as assigned.

Requirements:

• High school diploma or equivalent Required
• Associates degree or Bachelor’s degree Preferred
• Minimum of 2 years of experience in Home Health
• Fluent in English, with proficiency in speaking, reading, and writing.
• Bilingual in Spanish preferred
• Experience with Home Health Care Software required ie: DSL, HealthCare Synergy
• Strong understanding of OASIS and Plan of Care documentation requirements and regulations.
• Excellent attention to detail and analytical skills.
• Ability to work independently and prioritize tasks effectively.
• Computer Literacy ie: Microsoft Suite Proficiency
• Excellent Oral and Written communication skills

Schedule:

• Regular Full-time
• 40 hours per week
• Weekends as needed
• In-person work location in Van Nuys / Panorama City, CA

Compensation:

• Competitive pay ranging from $22.00 to $25.00 per hour based upon experience.
