JOB TITLE: Ambulatory Care Clinical Pharmacist

DEPARTMENT: Clinical Pharmacy

REPORTS TO: Senior Director of Pharmacy Services

SUMMARY:

Under general supervision, this full-time role provides clinical pharmacy services to clinicians and staff in primary care clinics. As a member of the clinical pharmacy team, this position is responsible for optimizing patients’ health outcomes through collaborative drug therapy management (CDTM) services via direct patient care, patient care team meetings, drug information and consultation, medication initiative rollout and clinician/staff education.

SPECIAL DUTIES AND RESPONSIBILITIES:

  • Regularly provides clinical, evidence-based pharmacotherapy services in the primary care setting through appropriate Collaborative Drug Therapy Management agreements and practice specific protocols.
  • Provides direct, effective, and individualized education and counseling to patients and/or family members/caregivers.
  • Performs formal and informal medication use consults with appropriate documentation.
  • Responsible for central clinical pharmacy support through participation in committees and workgroups.
  • Assesses need, develops and/or refines clinical pharmacy programs.
  • Assesses clinical and financial impact of efforts on an ongoing basis.
  • Participation in and/or management of standard department programs and services
  • Participation in the development of organization-wide drug therapy management materials and tools. 
  • Liaison work with one or two specialty departments.
  • Participates in staff development efforts in clinical areas.

QUALIFICATIONS:

  • Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required.
  • Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy.
  • At least two years of clinical pharmacy experience or 1-year clinical pharmacy residency required (preferably in ambulatory care clinical pharmacy).
  • Board Certification in Pharmacy is required (to be completed within two years of hire and upon achieving eligibility).
  • Must meet or be eligible to meet all criteria for CDTM per Massachusetts Board of Pharmacy regulations
  • Additional experience required if assigned to work directly on-site with Specialty clinicians as a primary role.

JOB KNOWLEDGE AND SKILLS:

  • Demonstrates understanding and commitment of mission and values of the health center
  • Effective verbal and written presentation skills.
  • Superior communication skills, including developing and giving presentations, both 1-1 and group.
  • Ability to work both independently and in a team
  • Knowledge of health plan drug formularies, 340B pharmacy program & medication savings programs.
  • Creative thinking and problem-solving skills.
  • Ability to supervise, advise and train clinical professionals and/or students in area of expertise.
  • Ability to understand importance of and respect for the confidentiality of all patient information in accordance with applicable standards and regulations.
  • Proficiency in Microsoft Office applications, including Excel, PowerPoint and Word.
  • Experience with EPIC preferred.

WORKING CONDITIONS/PHYSICAL DEMANDS:

  • Work is in a fast-paced clinic-based setting and remotely.
  • Transition between walking, standing and sitting.
  • Exerting up to 25 pounds of force occasionally in carrying, lifting, pushing, pulling objects.
  • Travel as deemed appropriate.
