Job description

JOB SUMMARY

Under the supervision of the Nursing Supervisor, the position requires the assisting of residents in activities of daily living and some housekeeping tasks.

JOB DUTIES and RESPONSIBILITIES

1. Assists residents in bathing, dressing, laundry, and other activities of daily living as indicated in the resident’s Individualized Service Plan.

2. Monitors a central call light panel and provides the necessary assistance and / or first aid in a timely fashion when necessary.

3. Ensures that residents are informed of all community activities and assists individual residents in participating in any activity that s/he may desire.

4. Maintains residents’ medications in a locked area, assists with medications as prescribed and records this appropriately. Assumes responsibility for the security of the medication room.

5. Makes rounds of the facility at regularly scheduled intervals to check on the safety of the residents and the facility.

6. Documents pertinent findings about residents and the facility in Eldermark Software, resident progress notes, resident observation forms and incident reports. Maintains resident daily census.

7. Works with Nursing, Case Managers, Resident Services Director, and the Assisted Living Administrator in reviewing resident’s needs such as reporting changes in condition and / or participates in establishing interventions for residents.

8. Performs general cleaning duties in accordance with an organized schedule.

9. Assists residents in maintaining independence: obtaining transportation and medical services; and completing weekly menus.

10. Upholds resident rights as stipulated in New York State Department of Health regulations, i.e., to protect and respect dignity and confidentiality of residents.

11. Develops and implements practices to minimize risk of personal injury, loss of life, damage to the building, furnishings, and equipment at Longview.

12. Performs other duties as assigned by Nursing and Assisted Living Administrator. Other duties as assigned, which may include but are not limited to: requiring the Resident Aide who holds a current NYS Home Health Aide certificate to perform tasks within that scope of care (tasks). In addition, a Registered Nurse may train a Resident Aide to perform a specific task which falls under the scope of tasks of a NYS Home Health Aide.

13. Participates in all pertinent risk management training and follows all Longview and department – specific risk management procedures as outlined in Longview Personnel Policy and Procedure Manual and the Policy Manual.

14. Reports all accidents / injuries to supervisor immediately and documents them on the “accident / injury report form.”

15. After becoming certified to assist residents with medications, must be able to carry out orders from physicians and follow through with accurate transcription of such.

16. Recognize changes in resident’s physical or mental condition and promptly report this to Nursing, physician or supervisor as appropriate.

17. Required to attend staff meetings and in-services as scheduled and maintain any job-related certifications mandatory and specific to job description.

18. Must uphold the Longview philosophy of treating residents and coworkers with respect and dignity.

19. Responsible for communicating with the physician(s), family member(s) / designated individual(s), emergency services, law enforcement, regulatory agencies and others as required and/or needed.

EDUCATION and EXPEREINCE:

Possesses a high school diploma or GED

Home Health Aide Certificate preferred

Medicine Technician Certification (will train)
