We are seeking a full or part time RN for Case Management.
Nurse Case Manager is responsible for telephonically and/or face to face assessment, planning, implementation and coordination of all case management activities with patients' to evaluate the medical needs of the patient and facilitate overall wellness.
The Nurse Case Manager will develop a proactive course of action to address issues presented to enhance the short and long-term outcomes, as well as, opportunities to enhance a patient's overall wellness through integration.
The office duties to include: writing care plans and facilitating comprehensive care for office patients. This position has full benefits and does not require nights or weekends.
