OVERVIEW

The Minimum Data Set (MDS) Coordinator plays a crucial role in a healthcare facility, particularly in long-term care settings like nursing homes. Their primary responsibility involves the day to day coordination and oversight of all aspects of the RAI and Care Planning process.

DAILY RESPONSIBILITES

• Conducting and coordinating resident assessments, which involve collecting and analyzing information on the physical, mental, and emotional health of residents.
• Completing and updating the MDS forms accurately and in a timely manner, as required by regulatory guidelines.
• Collaborating with interdisciplinary teams (nurses, therapists, social workers, etc.) to develop individualized care plans based on the information gathered from the assessments.
• Staying updated on federal and state regulations related to MDS assessments, ensuring compliance, and implementing any necessary changes to procedures to maintain adherence to these guidelines.
• Analyzing MDS data to identify trends, patterns, and areas for improvement in patient care. Generating reports and sharing insights with the healthcare team to enhance the quality of care provided to residents.
• Providing guidance, education, and training to staff members involved in the assessment process to ensure accurate and consistent data collection practices.
• Communicating effectively with residents, their families, healthcare professionals, and regulatory agencies regarding assessment processes, care plans, and any necessary updates or changes.

REQUIREMENTS

• Graduate of an approved LPN/RN Program.
• MDS Coordinator Experience Preferred
• Actively Licensed as an LPN/RN In assigned State of Employment
• High Attention to Detail
• Strong written and verbal communication skills.
