- 8am-4:30pm
- Occasional Weekends and Nights
- $25.25/hr DOE

- Makes initial visit with each hospice patient/family to assess spiritual needs and establish spiritual plan of care and bereavement plan of care.
- With patient/family/caregiver permission, contacts local clergy and coordinates spiritual care with them.
- Provides spiritual counsel and support to Hospice patients and/or families when indicated and requested.
- Serves as Hospice team member and attends and actively participated in all Hospice Interdisciplinary team meetings.
- Documents all client contacts or coordination with clergy. Turns in documentation to office within 24 hours.
- Tracks and records mileage on appropriate form.
- Assist families with funeral planning as they request.
- Offers spiritual services in a non-denominational way.
- Actively participates in Performance Improvement.
- Accepts accountability for own actions.
- Provides spiritual care to Hospice patients as requested.
- Responds to trauma alerts in person in the emergency department.
- Offers spiritual support to Hospice Care team including volunteers and other involved caregivers.
- Attends appropriate in-services and shares pertinent information with staff.
- Assists with Hospice training and in-services as needed.
- Treats patient information as confidential
- Speaks publicly on Hospice as requested and is comfortable speaking in front of a group.
- Completes Hospice training either in annual session, equivalent training from another hospice or through individual training sessions with Hospice Volunteer Coordinator.
- Provides spiritual care to patients in other hospital departments as needed.
- Works with Medical Social Worker to develop and make improvements to bereavement program.
- Solid background in Theology.
- Documentation free from active TB
- Valid driver’s license, auto insurance and reliable form of transportation
- Complete Livingston HealthCare annual educational competencies within assigned time frame.
- Education/background in bereavement/grief/loss counseling.
- Training in basic counseling techniques, crisis intervention and family dynamics is preferred.
- Interpersonal skills to develop trust of clients while respecting belief systems other than own.
- Previous experience with hospice or terminally ill.
- CPR certification.
- Ability to lead funeral/memorial services as requested by family.
