Summary:
A successful Enhanced Care Management Manager must be passionate about healthcare and want to make a difference in the lives of others while acting as a mission-driven catalyst to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families.
The primary duty of the Enhanced Care Management (ECM) Manager is to have a solid understanding of Enhanced Care Management and successfully guide a team of ECM Care Coordinators in identifying and assisting LCH patients and their primary care provider to provide better medical management and to track and gauge the effectiveness of that effort.
The ECM Manager will lead a team of ECM Care Coordinators who work closely with a multi-disciplinary team to develop and provide clinical services that are necessary to achieve an extended healthcare focus beyond the inpatient setting or traditional primary care of specialist office visits.
Essential Functions:
· Uses appropriate interpersonal skills to guide staff members to accomplish objectives; facilitate teamwork, participation, and cooperation. Fosters a work environment in which subordinates can develop. Provides necessary resources and guidance.
· Provides direction to the ECM Care Coordinators. Assures compliance with goals, programs, policies, and procedures.
· Engages with members across the Enhanced Care Management (ECM) Program.
· Monitor Care Plans in conjunction with provider recommendations related to chronic conditions and health needs.
· Develop health promotion and self-management training with input from members across departments.
· Establishes and maintains interpersonal relationships with both internal and external staff and other agencies.
· Work in conjunction with other case management staff and community providers in emergency and non-emergency situations.
· Participate as necessary to ensure continuity of care and follow through with care plan goals and needs are being met.
· Stay informed about referral interactions with behavioral health, food security entities, housing referrals, and other community resources.
· Can independently assess the psychosocial functioning needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient’s problems, strengths, weaknesses, coping skills, and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
· Enhances patient care outcomes, decreases costs, and improves patient satisfaction.
· Supports the overall needs of the health clinics by working flexible or extended hours when necessary.
· Supports the needs of LCH by traveling to all clinic sites as needed.
· Demonstrates competence with the mission, vision, and values of the organization to provide quality healthcare to those served in the community.
· Other work-related duties as assigned. Duties and responsibilities may be added, deleted, or changed at any time at the direction of leadership, formally or informally either verbally or in writing.
· Maintains confidentiality and respect for all sensitive information.
Required Education and Experience:
Education -
· Bachelor’s degree in a related field or equivalent work-related experience.
· Bilingual in English/Spanish preferred.
Knowledge, Skills, and Abilities -
· Commitment to the concepts of preventative health care program and team approach to health care delivery.
· Ability to provide effective guidance and training to subordinates, and project teams to support their success.
· Have the leadership ability to integrate multi-disciplinary teams.
· Ability to lead a team and follow through on assignments with minimal direction.
· Must have the desire to work with the public.
· Participate in QA/QI initiatives as required for overall organization improvement an patient experience improvement.
· Proficient with Microsoft products, electronic health records systems and other IT requirements
Job Type: Full-time
Pay: $66,560.00 - $87,000.00 per year
