We are looking to hire a LVN Home Health nurse to join our team! The Home Health LVN will be responsible for the following job duties, including but not limited to:

• Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
• Performs IV therapy, wound care, and wound vac therapies.
• Provides patient and caregiver education.
• Provides care using infection control measures that protect both staff and patient.
• Assures the continuity of care through delivery of quality patient care.
• Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.

Pay starts at $65k per year. Benefits provided.
