Living Innovations is Hiring a Community Living Coordinator Near Dover, NH

We accomplish this by putting the people we support at the center of all we do and by providing excellent customer service to all team participants including guardians, case managers and family members. Travel during the work day will be expected as this position is responsible for bi-monthly home visits.

What will you be doing?

  • Creating and implementing individualized service plans.
  • Ensuring quality delivery of all services and programs for individuals supported in your region.
  • Conduct bi-monthly in-person home visits to provide support and ensure the quality of shared living services.
  • Maintaining positivity, productivity and overall morale of staff while ensuring satisfaction for the individuals and families supported.
  • Providing leadership, supervision, professional development, and growth opportunities to all assigned staff.
  • Maintaining records, data, and information in adherence with Living Innovations policies as well as State and Federal regulations.

QUALIFICATIONS:

  • High school diploma or equivalent.
  • Bachelor’s degree preferred but not required.
  • One year of experience in this or related position in a social service or healthcare setting. DSP experience preferred.
  • Must pass comprehensive background checks per state and company regulations/policies.
  • Valid state issued driver’s license, current vehicle insurance declaration and vehicle registration.

Job Type: Full-time

Pay: From $20.00 per hour
