Rose View Nursing and Rehabilitation Center is seeking to hire a compassionate Care Concierge to support our nursing team! This is a Full Time 11am-7pm position, including every other weekend and every other Holiday.

Qualifications
• Demonstrate passion & enthusiasm to work with elderly Residents
• Minimum high school diploma or equivalent
• Excellent interpersonal & communication skills
• Ability to multi-task as well as stay on task & concentrate with constant interruptions
• Must be a Self-Motivator
• Excellent customer service skills
• Ability to adapt to change
• Team Player with positive attitude
• Pass the PA criminal history check, pre-employment physical, and Drug Screen
Responsibilities
•Treat each resident with care, compassion, and respect
•Communicate effectively with residents and their families as well as all departments
•Embrace our person-centered care approach to meet each resident's needs and preferences
•Serve a vital role on our team by acting as our residents' spokesperson
•Work under the direction of the Admissions Director
•Greet new admissions and families and answer any questions that may arise
•Communicate any observations in change of resident's status to the nursing team
• Welcome new Residents & their families to ensure a smooth, hassle-free transition
•Ensure rooms are clean & ready before welcoming new Residents
•Introduce new Residents & their families to our "Home"/show them around the Facility
•Escort new admissions to the appropriate room and provide “tuck in” service ensuring all needs are met
•Complete Admission Paperwork with residents and families upon admission as assigned
•Coordinate between new residents & unit managers or RN supervisor to ensure their quick evaluation upon arrival
•Create inventory of resident belongings upon Admission/ Label Clothing
•Coordinate with new resident’s assigned CNA to ensure quick check-in upon arrival
•Follow up with new residents and family within 24 hours to check on stay
•Hand out food trays during lunch and supper meal pass
•Answer call bells and pass ice water as needed
•Communicate resident needs to interdisciplinary team
•Make rounds daily on both floors to discuss Residents’ needs and concerns
•Maintain a log with questions, concerns, and issues raised; and any follow-up steps are taken
•Submits logs to the Facility Administrator
•Assists Residents with the discharge process and ensure they are given a warm farewell.
•Provide farewell gifts, Follow up the following week, send card and follow up call within 6 weeks of discharge.
•Complete surveys with discharging short stay residents
•Apply resident names to rooms, check that resident names are on doors
•Assist with room changes as needed
•Put together and deliver welcome gifts
•Assist with special events and marketing as needed
•Cover reception as needed
•Social Media Ambassador- Take pictures of facililty events, create captions and submit posts to our facility social media pages
•Help where help is needed, be a team player, and assist in any problem area
