LiveOnNY is Hiring a Sr. Family Support Advocate Near Long Island, NY

Overview

At LiveOnNY we save lives, provide comfort, and strengthen legacies through organ, eye and tissue donation for the 13 million New Yorkers we serve.

 The Senior Family Support Advocate provides onsite response, is responsible for advocating for donation, and supports families through the organ and tissue donation process. Initiates timely donation conversation with families either in person or telephonically and in collaboration with hospital partners. Maintains authorization rates that are equivalent or exceeds goals set by the organization. Acts as preceptor in the field and participates in the development of new staff. Supports and fosters the Mission, Vision and Core Values of LiveOnNY and its commitment to organ and tissue donation

Responsibilities

  • Ability to perform all essential job duties of the Family Support Advocate. Evaluates and determines bereavement support needed for potential donor families throughout the entire designated service area.
  • Responsible for educating potential donor’s authorized person about donation options, status of donor registry, or requesting authorization for donation from the authorized person.
  • Demonstrates sensitivity to diverse populations and their beliefs. Remains culturally competent through continued education opportunities.
  • Obtains authorization or provides notification with first-person authorization and conducts a donor risk assessment questionnaire with the appropriate surrogate of the potential organ donor.
  • Ensures effective communication and strong collaboration with donor families, hospital personnel, administrator on call, physicians, related donation agencies and other LiveOnNY departments. 
  • Collaborates with hospital staff and on-site clinical staff. 
  • Follows directive from Referral Administrator and Administrator on call (AOC), to discuss the opportunity for donation with potential donor families after the authorization trigger is met.
  • Actively leads or participates with clinical staff in all required huddles (i.e., Care team briefs, preauthorization huddle, post authorization huddle) providing critical information related to family dynamics, identified support system and or outcome of family conversations.
  • May serve as a first responder on organ referrals; responsible for collecting basic information from the potential donor’s chart and providing that information to Clinical Services.
  • Completes all required documentation accurately and in a timely manner related to first responding responsibilities and family conversation.
  • Acts as preceptor in the field and participates in the development of new ongoing Family Support Advocates and Clinical Coordinators. 
  • Refers donor families to LiveOnNY aftercare services program, community agencies and provides counseling referrals as requested and appropriate.
  • Demonstrates leadership skills by leading or actively participating in department activities and special projects (approved by family service leadership) quarterly, that contribute to advancing the authorization goals of the department.
  • Assists the department in providing effective on-boarding and annual training programs as needed. Assists in the onboarding of travelers on an as needed basis.
  • Required to serve as “Senior On-Call” for overnight emergent phone approaches, high priority, or level one referral onsite family needs, including but not limited to emergent approaches, DCD OR family support, Honor Walk support, etc.

Qualifications

  •  Two years’ experience within an Organ Procurement Organization and at least one year at LiveOnNY preferred. BA/BS in counseling, psychology or social services, crisis intervention, or education or five years’ experience in organ procurement consent, with a proven track record, required. Excellent knowledge of the dynamics of grief and bereavement. Experience in trauma/critical care setting helpful. 

  • Master’s degree in social work, or related field, is preferred. 

  • Knowledge of the principles of critical care. Knowledge of and ability to function in hospital setting. Ability to work with individual(s) in a bereavement situation, good interpersonal skills. Ability to work as a team member.
  • Experience and good practice/performance in preceptorship and teaching abilities directly related to the job functions of Family Services department. 
  • Must possess a valid driver’s license. Must own or have immediate access to a motor vehicle duly registered and insured by law.

Salary Range: $79,081 - $98,851 per year
