Little Treasures Early Intervention and Case Management agency is dedicated to the wellness and enrichment of children and adults. The agency strives for excellence and resilience when employing case managers. At this time, we are seeking to hire case managers for all boroughs (Brooklyn, Bronx, Queens, and Manhattan).
A Case Manager assess, monitors and advocates for the services needed by the client and the family as a whole. Case Managers assist clients with obtaining resources within their community to achieve a quality of life they wish to have and help reduce the amount of hospitalizations.
Below is the description of a case manager's responsibilities:
-Create a Plan of Care to address client's needs
-Collaborates with client’s care team to ensure that the client is getting the adequate care needed to manage their medical conditions
-Medication Management
-Link clients and families to crisis counseling, therapeutic services, and safety interventions
-Assists with applying/obtaining benefits/entitlements (HRA benefits, SNAP, WIC, Social Security, Food Pantry, Access-A-Ride, etc.)
-Maintaining weekly contact via telecommunications with the client and their family to address their needs
Required:
-Associate Degree or Bachelor Degree in Social Work, Psychology, Sociology, Human Services and/or related field (with 2 years experience is a plus) OR
-Master's Degree in related field (with 1-year experience is a plus)
-CANS-NY certified upon hire(Required)
-Spanish speaking (Preferred)

Ideal candidate may have previous experience as - Case Manager, Case Management, Adult Care Manager, Social Worker, Care Manager
