Charge Nurse (RN or LPN)
JOB OBJECTIVE: The Charge Nurse is responsible for providing and supervising the medical and personal care services delivered in their assigned area. The stated goal of the Registered Nurse is to assure resident safety and attain the highest practicable health and psychosocial well-being of each Resident is attained and maintained in accordance with the comprehensive assessment and plan of care. These services are consistent with the Home's standards and philosophy and are in compliance with all regulations, codes, policies and procedures that govern the Home.
MISSION: The Charge Nurse will carry out their duties in keeping with the mission of Little Sisters of the Poor, namely caring for the aged poor with love, respect and dignity. “Making the elderly happy, that is what counts” was a saying of the Little Sisters of the Poor foundress Saint Jeanne Jugan. Central to this philosophy is the promotion of a holistic approach for our Residents in an atmosphere of kindness and respect in creating a family spirit.
JOB STANDARDS: The Charge Nurse must have:
  • A license to practice in the Home’s state as a Registered Nurse or Licensed Practical Nurse
  • The ability to function as the nurse in charge during their shift.
  • Possess good written and verbal communication skills, including the ability to read, write and understand English.
  • Be able to function independently and perform related duties/tasks with a minimum of supervision or direction.
  • Good observational skills and a working knowledge of the nursing process.
  • The ability to supervise others and/or have prior management experience.
  • The necessary leadership skills to interact effectively with staff, Residents and families.
  • The ability to perform *essential job functions with or without reasonable accommodations.
JOB RESPONSIBILITIES AND DUTIES The Charge Nurse will:
  • *Observe and monitor assigned Residents daily and report current status to the relief nurse.
  • * Timely reporting of changes in Residents status or condition to Director of Nursing or their designee.
  • *Notify Resident physician and responsible party of change of Resident status, with prior reporting (if possible) to Director of Nursing or Mother Superior.
  • Assist Resident with issues related to psychosocial needs, i.e. aging and health changes/illness, loss and grief.
  • *Administer and document prescribed medications/treatments accurately and timely and in compliance with policies/procedures.
  • *Respond calmly and quickly to emergencies involving Residents or employees and report the situation to the Director of Nursing or her designee.
  • *Transcribe accurately and timely all physician orders.
  • Participate in the comprehensive Resident assessment and update the plan of care in a timely manner to assure its accuracy and appropriateness.
  • Prepare for and assist with Physician visits to the Home.
  • Assist with orientation of Nursing Assistants and/or select nursing personnel as indicated.
  • *Provide necessary supervision and follow up of care performed by Nursing Assistants.
  • As required, participate in the performance evaluation of Nursing Assistants.
  • *Report to the Little Sister, Director of Nursing, or her designee any unresolved or pertinent issues with Nursing Assistants.
  • Assure follow up of rehabilitation/restorative measures.
  • *Communicate daily all pertinent Resident care information to Nursing Assistants, Nursing Staff, and the Director of Nursing.
  • *Communicate with family/responsible party regarding Resident issues/status as indicated with the assistance of the designated Little Sister of the Poor and/or Director of Nursing.
  • *Prepare accurately and timely documentation, including but not limited to accident/incident reports, admission/readmission, discharges, changes in Residents conditions/status, etc..
  • Monitor medical supplies and equipment to ensure proper use and safety.
  • Strive for optimal use of supplies/materials, keeping in mind cost and budget limitations.
  • *Maintain confidentiality of all Resident, departmental and organizational information.
  • *Protect Residents rights and personal healthcare information.
  • Work effectively and cooperatively as a team member within the department and with all departments or other disciplines.
  • Have the desire to work in the area of geriatrics, rehabilitation, and/or psychiatric nursing.
  • Acquire the basic knowledge of State and Federal regulations and guidelines governing long term care.
  • Observe dress and conduct codes, including wearing identification badge.
  • Meet attendance standards as established and provide proper notice of absence.
  • Complete all assignments and duties in prescribed time.
  • *Follow all safety, security, infection/exposure control and hazardous waste policies and procedures (including Category I universal precautions).
  • Attend required in-services and department meetings to continually refine clinical skills.
  • Perform other duties as assigned by supervisor.
  • Offer flexibility with the schedule to meet the needs of the Nursing Department.
    WORK SETTING/ENVIRONMENT: The Charge Nurse will perform work primarily indoors, but may attend meetings/seminars outside of the facility or may participate in outdoor activities of the Home. The work environment is well lighted, clean and suitable to tasks performed by the employee within temperature control standards that govern a skilled care facility. The potential at risk factor pertains to Category I exposure to blood, body tissue, fluids, with occasional exposure to hazardous materials and bodily injury/illness. Seldom are there exposures to toxins.
    EQUIPMENT OPERATED: The Charge Nurse will be expected to operate a variety of equipment, i.e. medication cart, stethoscope, blood pressure apparatus, blood sugar testing devices, nebulizer apparatus, thermometer, needles, syringes, telephone, pacemaker equipment, oxygen/oxygen concentrator, medications, I.V. equipment, Resident lifts, and computers. *This list is not all inclusive
    PHYSICAL AND COGNITIVE REQUIREMENTS TO PERFORM THE ESSENTIAL JOB FUNCTIONS: The Charge Nurse is able to:
  • Perform simple motor skills such as standing, walking, sitting, etc., and difficult manipulative skills such as, equipment calibration, etc.
  • Perform tasks which require hand-eye coordination such as injections.
  • Remember simple and multiple tasks and assignments given to self and others ranging from short to long periods of time, and to concentrate on moderate to fine detail with constant interruption attending to tasks/functions for more than 60 minutes at a time.
  • Have sufficient vision to discriminate color to observe a Residents' condition, read care plans, charts and other documents, read call light panels, instructions, color coded medication boxes and labels, thermometers, etc.
  • Push occupied medical carts and wheelchairs with weight capacity more than 200lbs. 
  • Able to frequently lift and move objects weighing 50lbs, and occasionally lift and move objects weighing more than100lbs with proper technique, ergonomics, and supplemental equipment. This includes the occasional positioning and moving of Residents from all positions. In emergency situations, must be able to bear entire weight and move Resident to safety.
  • Hear, auscultate, and interpret heart and lung sounds.
  • Have the cognitive ability to constantly understand theories behind several related concepts, comprehend, remember and follow multiple task/directions over long periods of time.
  • Have the emotional stability to deal effectively with stress created by Residents' illnesses, disabilities and the aging process, organizational change, and working cooperatively as part of the health care team.
  • Walk and stand constantly, frequently climb stairs, squat; kneel; twist; sit; bend and reach from floor to overhead.
  • Have sufficient vision and color discrimination to observe a Residents' condition, read care plans, charts and other documents, read call light panels, instructions, color coded medication boxes and labels, thermometers, etc.
  • Adequately hear verbal instructions, answer phone calls and call lights, respond to emergency signals/alarms and communicate effectively with Residents and others.
  • Perform tasks which require finger, hand, and arm dexterity/mobility and hand-eye coordination to safely provide personal cares, treatments, medications.
  • Able to speak clearly to report Resident conditions and communicate effectively with Residents and others.
  • Constantly understand theories behind several related concepts, comprehend, remember and follow multiple instructions/directions given to self and others, frequently amidst noise and constant interruption while concentrating on fine detail and tasks for more than 60 minutes at a time.
  • Emotionally capable in effectively dealing with stress created by Residents' illnesses, disabilities and the aging process, organizational change, and working cooperatively as part of the health care team.
