EIS Housing Navigator
POSITION SUMMARY
LRMC's EIS (Early Intervention Services) team aims to provide access to health care services and medication for persons with HIV/AIDS (PWHA) and to strengthen skills to achieve optimal treatment adherence with the goals of viral suppression and reduced HIV-related mortality.
The Early Interventions Services (EIS) Housing Navigator will work closely with the EIS Case Managers in supporting the needs of PWHA households in securing and maintaining stable housing, resulting in improved health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:
  • Work with Case Managers in meeting the various housing and financial needs of those applying for Housing Opportunities for Persons Living with HIV/AIDS (HOPWA). 
  • Review HOPWA referrals received from case managers, when patient is not eligible for HOPWA will work with patient to find assistance within other agencies that patient may qualify for.
  • Assess eligibility for housing assistance for PWHA who are experiencing financial hardship(s). Including but not limited to experiencing a recent loss or reduction in income or an increase in expenses.
  • Complete initial HOPWA application(s) and update annual housing assistance paperwork for all patients who are enrolled in housing subsidies.
  • Accompanies patients to seek housing placement opportunities that address applicants’ financial budget and needs. Will transport patient if needed.
  • Assists patients as needed with applications for housing.
  • Inspects living quarters to assure the funding source standards are met.
  • Submits check requests in a timely manner to EIS Services Coordinator.
  • Submits monthly and quarterly patient lists to EIS Services Coordinator to review ongoing eligibility. 
  • Provides needed advocacy and support with housing integration and retention issues that may arise; addressing ongoing issues that have the potential to hinder the patient’s overall health and well-being and /or housing stabilization. This could include case consultations, property, and care team conferences.
  • Providing advocacy as needed for patient with property management; to ensure a successful housing placement as well as responding property complaints, enquiries, and questions.
  • Incorporation of life skills training including budgeting, housing, employment, and more.
  • Coordinates emergency financial assistance for rent and utilities when patient is eligible.
  • Develop and implement a housing stability action plan for each patient and update and monitor progress as needed or required by funding guidelines.
  • Have a working knowledge and understanding of the impact of the current housing crisis in this region, along with working knowledge or training related to the Landlord /Tenant Laws of South Carolina.
  • Document activities, plan of care and results in the electronic medical record in an effective manner while strictly adhering to the policies and procedures in place.
  • Educate patients on how to access services at LRMC and be responsive to patient/families/caregivers needs and preferences.
  • Provide culturally and linguistically appropriate support, guidance, and encouragement for the patients/families/caregivers.
  • Provide integrated healthcare services by sharing patient information in accordance with HIPAA and Confidentiality laws within and outside LRMC to support an integrated and coordinated patient treatment plan.
  • Participate in regular staff/team meetings, trainings and other offsite meetings as requested, and proactively supports PCMH initiatives related to care coordination.
  • Proactive member of care teams in team-based care initiatives to optimize results and ensure high quality patient-centered care as part of the medical home.
  • Should spend between 50 and 80 percent of their time out in the communities that s/he serve to ensure connection with housing resources and linkage to housing services.
  • Perform other duties as assigned.

  • Must be able to work a very flexible schedule including late hours and weekends.
  • Be self- motivated and task oriented.
  • Must demonstrate experience and an understanding of the difficulties that come from working with marginalized populations that brings their experience that can include chronic homelessness, past or current substance use/abuse and ongoing mental health instability.
  •  Be able to develop and maintain strong professional community relationships.
  • Be able to develop and maintain supportive relationships with all patients and families that are affected by HIV/AIDS.
  • Be comfortable and non- judgmental when working with patients that are homeless, use injection drugs and LGBTQIA .
  • Strong work ethic and reliability.
EDUCATION and/or EXPERIENCE
  • High school diploma or Equivalent is required, bachelor’s degree in social work or in related field preferred.
  • Two years of proven experience in health or social services related area. Experience working with homeless individuals, or people living with disabling conditions, including severe and persistent mental illness or HIV/AIDS required.
