Long Island Select Healthcare Inc. (LISH) is a comprehensive network of community health centers under a Federally Qualified Health Center (FQHC) model. We provide Article 16 and 28 clinic services at 8 locations across Suffolk County, New York.

LISH is currently searching for a Nurse Practitioner or Physician Assistant in Family Medicine who will report to the Chief Medical Officer. We are looking for energetic individuals who will have the opportunity to work with LISH's experienced clinical staff. A successful candidate will be an experienced and confident professional, they will possess strong communication and analytical skills. General duties include:

• Provides comprehensive care to patients in an ambulatory care setting.
• Provides consultations by examining patients; reviewing medical records, reports, and logs; researching information and developing conclusions and care recommendations.
• Adhere to professional standards, practice policies and procedures, federal, state, and local requirements, and regulatory standards.
• Works as a team player assisting other staff providers with developing diagnosis and patient treatment options.
• Prescribes or administer treatment, therapy, medication, and other care to treat or prevent illness, disease, or injury.
• Orders, performs and interprets test and analyze records, reports and examination information to diagnose patients’ condition.
• Records and maintains patient information such as medical history, reports and examination results. Monitors patients’ conditions and progress and re-evaluation treatments as necessary.
• Reviews health risk assessments (HRA’s) to help determine a plan of care.
• Explains procedures and discuss test results or prescribed treatments with patients.
• Refers patients to medical specialists or other practitioners when necessary.
• Updates job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
• Contributor involved with LISH's PCMH Initiative, Chronic Care Management Program, Quality Initiatives, telemedicine opportunities, and more!

Requirements:

• Graduated from a regionally accredited school
• Licensed & Registered to practice in NY by the NYS Department of Education
• National Certification Required (i.e.: ANCC or AANP, or NCCPA)
• New graduates are welcome
• Bilingual Preferred
• Ability and means to travel to other sites for coverage as needed
• Scheduling Software and EMR experience, eClinicalworks preferred
• BLS certified
