Job Goal: The job of Licensed Physical Therapist is done for the purpose/s of assessing students’ functional physical development level; planning, implementing, and evaluating appropriate treatment/s to meet individual student objectives (Individualized Educational Plan or 504 Plan); providing information to family and school personnel for program development and student placement; and providing information to instructional personnel on specific student issues.Duties and Responsibilities: •Evaluate children using appropriate testing protocols, tools, etc. to follow funding guidelines for each particular child.•Assess identified students to determine whether there is an educationally significant need for physical therapy.•Create appropriate goals/objectives to be included in each evaluation.•Provide therapy services to children based on the recommended number of units from evaluations and prescriptions.•Assist students in gaining functional independence to benefit from the educational environment more completely.•Document all therapy services by locking and electronically signing weekly progress notes.•Update and turn in efficiency reports weekly.•Complete additional paperwork requirements accurately and timely as notified by the administration. These requirements include, but are not limited to: Annual re-evaluations, Discharge summaries•Collaborate with administration and other disciplines to ensure quality services for children and families.•Communicate effectively with district personnel, families, and the community.•Support the value of education•Keep abreast of new information, ideas, and techniques in pediatric physical therapy and practice in educational environments.•Travel between locations where students require services•Comply with all district policies, rules and regulations•Perform other related work as required
