Intake Coordinator

Link Home Therapy is looking to hire a motivated, driven and professional Intake Coordinator to join the team.

Main objective of this role is to welcome, register and admit patients in our assisted living facilities and home care locations.

Duties and Responsibilities

· Complete admission paperwork with new patients and referrals

· Review and verify patient insurance coverage and benefits

· Schedule follow-up and tracking system to ensure timely setup for services

· Coordinate staffing for cases/ schedule appointments for therapy

· Maintain open communication with internal team and family members

· Perform administrative and clerical duties as necessary by team

· Conduct satisfaction check-ins with patients

· Track referral conversions/ provide reporting to referral sources

Qualifications

· Experience with healthcare admissions or medical administrative duties is preferred

· Experience with MS Office

· Strong organization and communication skills

Location: Full-Time on site in Englewood Cliffs, NJ
