SUMMARY: The Social Service Worker helps clients by assessing their situation; setting goals; obtaining required services. They establish course of action by exploring options; setting goals with client and obtains assistance for client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
·Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education. ·Refer individuals to educational or work programs. ·Refer clients to community or social service programs. |
