**Job Overview:** We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting assessments, and developing care plans to ensure optimal outcomes. **Responsibilities:** - Conduct patient assessments including vital signs and medical history review - Collaborate with healthcare providers to develop and implement individualized care plans - Monitor patient progress and update care plans as needed - Ensure compliance with HIPAA regulations and maintain accurate medical records - Coordinate discharge planning and post-discharge follow-up care - Provide direct patient care in various settings such as ICU and ER - Perform case management duties to facilitate continuity of care **Requirements:** - Valid RN license and BSN degree preferred - Proven experience in hospital settings with strong knowledge of patient assessment and case management - Familiarity with ICU and ER procedures and protocols - Ability to develop comprehensive care plans tailored to individual patient needs - Proficiency in maintaining accurate medical records and ensuring HIPAA compliance - Strong interpersonal skills for effective communication with patients, families, and healthcare team members This position offers the opportunity to make a meaningful impact on patient outcomes while working in a dynamic healthcare environment.

Pay: Up to $95,000.00 per year
