Case Manager/QMHS - Community Juvenile Justice
Lighthouse Youth & Family Services has a Qualified Mental Health Specialist/Case Manager position open in Community Juvenile Justice located in Hamilton County. As part of a mission focused and innovate treatment program, the Case Manager will The clients served have involvement in the county juvenile justice system and are enrolled into Lighthouse for diversion, prevention or re-entry services. This is a full-time position and the candidate must be willing to work a flexible schedule between the hours of 9:00am and 6:30pm for best client service delivery.
Competitive pay based on education and experience.
Starting Hourly Rate: $19.04
Essential Functions:
Provide community-based mental health case management services to adolescents and young adults.
work closely with other professionals in providing treatment services in the areas of mental health, employment, education and criminal offending behaviors.
provide community-based services
facilitate groups, coordinate service learning projects,
develop treatment plans and maintain accurate and current documentation for client files.
Qualifications:
Bachelor’s Degree in Social Work or related field required.
At least 6 months of experience working with youth in a social service setting required.
Experience with adolescents and young adults with mental health and criminal offending behaviors preferred.
Experience with facilitating groups is preferred.
Must have excellent written and verbal communication skills.
Must have a car and good driving record.
Bilingual and/or Sign Language proficiency a plus.
