This position is working with intellectually and developmental disabilities adults in a residential program.

The number of openings: 1

Yearly Pay Rate: $42,085.00

Hours: 40-hour shift

Job: 220

Sunday (1pm - 10pm)

Thurs (1pm - 10pm)

Friday (2pm - 10pm)

Sat (8am -10pm)

What You’ll Do:

Collaborate with an exceptional team to plan and engage in fun activities. Help individuals lead more independent and secure lives in their community. Provide support through case management, life skills coaching, and advocacy.

RESPONSIBILITIES:

  • Promotes optimal health for individuals through routine, preventive, and emergency health care.
  • Responsible for assisting with all hands-on direct care which includes: showering, toileting, shaving, brushing teeth, meal-time assistance (some one-on-one assistance with eating during mealtime)
  • Ensures that residential environments are safe and secure and where individuals are free from harm.
  • Provide education training, counseling, and physical assistance when necessary to achieve ISP goals in the following areas: functional academics, domestic skills health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
  • Develop, write and present annual ISPs and semi-annual assessments in consultation with other residential staff and directors.
  • Provides transportation to individuals as necessary
  • Communicate with persons significant to the individual served (i.e. family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.
  • Performs case management duties including, maintenance of individual confidential files, ensuring cases are reviewed, documentation of activities, and training. Attends regular meetings as necessary.

Job Responsibilities: (Case Management duties) & (Direct Care Work)

  • All-Male residents (five)
  • High functioning
  • There will be some direct personal care!
  • Tracking and recording ISP data
  • Writing Monthly progress notes
  • Attending ISP meetings
  • Completing ISP assessments
  • Assisting with appointments and all follow-up
  • Serving as a contact for other providers (Guardians, Day Program staff, etc.)
  • Cooking, assisting with feeding, etc.
  • Hands-on support with toileting
  • Showering
  • Changing briefs, and colostomy bag if needed!
