This position qualifies for the Oregon Health Care Provider Loan Repayment Program and Public Service Loan Forgiveness!

ACT is an intensive Assertive Community Treatment fidelity program characterized by high staff to client ratio and limited caseload. This is a community-based program, with approximately 80% of your time spent in the community. Our team works with adults, ages 18-99, who have severe and persistent mental illness who need intensive, wrap around services to assist with issues including (but not limited to) difficulty with housing, social support, interactions with law enforcement, addictions, economic status, employment, distress tolerance, emotion regulation, interpersonal skills, basic needs, and general independent living skills that are barriers to residing independently in the community.

The Licensed Medical Provider provides psychiatric assessment and management for individuals enrolled in the ACT program, a multidisciplinary team approach with assertive outreach in the community for clients experiencing the most severe symptoms of mental illness and integrated care services in conjunction with primary care delivered by community partners. ACT programs are evidence-based, and fidelity reviewed to help support those clients at highest risk of psychiatric crisis and hospitalization and involvement in the criminal justice system. Services will be provided in both onsite and in the community.

Essential Responsibilities:

  • Provides psychiatric evaluation for clients including gathering and maintaining patient information and records, including social and medical history obtained from patients, relatives, and other professionals. Analyzes and evaluates patient data and test findings to diagnose nature and extent of mental disorder.
  • Provides information and education about pharmacological factors in the treatment of and service of clients to other agency staff and the general public as needed.
  • Provides ongoing medication management and monitoring including monitoring for side effects and therapeutic drug levels.
  • Provides consultation with Primary Care Providers, LifeWorks NW therapists, social workers, psychologists, Psychiatric Nurses, Nurse Practitioners and treatment team when indicated.
  • Maintains chart notes in electronic health record system within 24 hours of patient care.
  • Provides medical supervision of client treatment plans including review and signature.
  • Advises and informs guardians, relatives, and significant others of patients' conditions and treatment.
  • Meets standards for documentation and work accuracy as defined in the Performance Evaluation including timely completion of progress notes, assessments, treatment plans, authorization requests.
  • Travels to see clients in the community.
  • Teaches, takes continuing education classes, attends conferences and seminars to increase understanding of mental, emotional, and behavioral states and disorders.

Qualifications:

  • Masters degree in Mental Health Nursing with Psychiatric Mental Health Nurse Practitioner (PMHNP) license and prescriptive privileges.
  • Good computer skills including a functional knowledge of Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system.
  • Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
  • May require access to a car and valid driver's license.
